Back in September of 2022, West Lake RX LLC issued a fraudulent claim to Medicate in my name and ******** account number for a "therapeutic continuous glucose monitor" and "all supplies" in the amount of $351.90. I did not receive a notice of this from ******** until a few days ago, and ******** approved the charges and paid $234.60, the remainder being left as my responsibility. I am retired, aged 83, and live in ****, *****. I do not know how West Lake RX got my name; I have never bought anything through them and have not been in ***********, where West Lake RX is located, in over 12 years. I also received a bill today from Cigna, my supplemental insurance, for $46.92. According to my Medicate statement, ********************************** was the person who made the fraudulent purchase in my name. I am disgusted that ******** was cheated and that I also was cheated. I will contact Medicate tomorrow and report this problem. Thank you.Initial Complaint

This fraudulent company billed my ******** account for $711.73 for a blood sugar monitor and supplies which we never ordered or received. Apparently they have been doing this to many people according to Yelp reviews. We have reported the fraud to ******** and our supplemental insurance company and would like to send a warning to other potential victims.
