I first started to go to Endeavor Physical Therapy located at ********************************************************************* on January 12th 2022 for a hand injury. I attended physical therapy 17 times between January 12th 2022 to March 17th 2022 as seen on Exhibit A. I paid Endeavor Physical Therapy a total of $583.76 (reference Exhibit A, page 2 with attached receipts) which I was told was after insurance had already paid Endeavor Physical Therapy. I talked with ********** Blue Shield of ***** and was told they were also billing the insurance provider, essentially doubling charging for services provided. I went in person to Endeavor Physical Therapy on April 18th 2022 to discuss the billing discrepancies and discovered that the $369.62 that I had paid Endeavor Physical Therapy was not applied to my **** correctly as they billed myself and the insurance provider and was told this would be corrected. I also followed up with an e-mail on April 21st 2022 after my **** was not yet corrected. On April 22nd 2022 I was reached out to by ********************* informing me that I had overpaid by $233.18 (reference *********) and she will apply that amount to the total balance (total balance reference Exhibit C and Exhibit B shows an itemized break-out) I still owed which totaled $214.14 (reference Exhibit D). I then paid the $214.14 in full on my Amex card (reference Exhibit A, page 3). I was informed that my account was paid in full and I didnt owe them anymore money. I want to note that this process was a struggle as Endeavor Physical Therapy kept attempting to **** my insurance and myself for services rendered. I also want to note that *************************** was seen shredding all of my receipts and had excuse after excuse on why my insurance and myself were being double billed. On May 20th, 2022 I received another **** from Endeavor Physical Therapy saying I still owe them $252.24 and showing that the insurance paid nothing, thus far on my account (reference Exhibit F).
