According to the Code of Ethics provided by the ******** Dental Association Code of Professional Conduct : A dentist should abide by the put the patient-first promise. The code reminds all dentists that they have the duty to put the patient first, to involve the patient in treatment decisions in a meaningful way and to treat the patient taking into account the patient's desires, as long as those desires are within the bounds of accepted treatment. The method of their treatment plan has left me with having to pay twice for services already performed. The crown that was placed on my tooth has damaged my gum and has to be replaced. I was not provided the cleaning that was much needed that caused further build of bacteria. The filling that was done was not complete and my tooth continues to decay. When I tried to schedule a deep cleaning that routine I was told I had to get a crown first. This is unheard as my new dental team has advised me. Dentist regularly perform cleaning first to rid the mouth of bacteria which will undo pervious work performed. My fillings and crowns were done less than 2 months ago. I went to a new dentist today because they were able to fit me in for a cleaning immediately. During my comprehensive exams I was told that all the previous work was bad. My mouth (fillings crowns) have to be redone. I am requesting a full refund for all services provided.Business response

Based of information received, with patients name, date of birth, and phone number, patient was not found as a patient of record at our office. At this time request for complaint to be dismissed.Customer response

My former name was *************************** , the office failed to update update although they were informed of this. They email me @***********************. I'm sure if they would would have searched using my email they would have found me.Business response

As per patients request full refund has been made to patient and to her Delta Dental insurance company for dental services completed.Customer response

Upon calling my insurance I found out they have not received the refund. I did receive my refund within a few days.
