This all started with the phone call from the intake person, *****. I missed her call so I called her back. She kept replying as if I was bothering her for returning her call. My mom was moved there and no one in the family received a phone call to know she had made her arrival. Once there, she asked for an assistive device to use the restroom. She was told no. During med pass, on another morning, the nurse offered medicine to my mom. My mom is very competent when it comes to her medications. My mom simply asked what medicines she was being given. The nurse became combative and states she will report my mom is refusing her medication. I called to check on my mom and was assured I would receive a phone call with an update. I called 02/22/2024 @ 3:03pm. As of 02/23/2024 @ 12:15pm, I still have not heard from anyone. Inside room 206, the toilet seat is broken. That is a liability. The trash is over running. Spoke with the social worker, *****************************, and she did not seem moved at all. She basically told me she was getting all the paperwork together and will have it ready. She NEVER asked what was wrong or why we were wanting an early discharge. As if she was not concerned or as if she was familiar with the actions of the staff already. Oh and the food. Burned, over cooked, and tasteless are some of the adjectives given. My mom was served green beans that looked like mashed potatoes with food coloring. Nonetheless, the actions of the staff have made us arrive at the decision of having my mom discharged from this facility. I would hope no claims are filed on her insurance.Business response

Good evening,
We received your correspondence regarding this complaint in the mail yesterday, postmarked 3/29/2024. We never received any other correspondence. Please note my email for future correspondence: sg-********************************** My Cell # is ************. The office # is ************. The address on the envelope with your letter is correct.
It is unfortunate this family member felt or perceived her mom's experience in our community in a negative manner. While she only stayed a few days and discharged, we did receive a formal grievance and were working to rectify any concerns internally.
I would like to provide a formal statement and include documents and notes to support our response.
How much time do I have to do this at this point?
Thank you in advance for your follow up.
Regards,
