My doctor sent orders over to this ****** back in November in which they failed to properly submit my claim to my insurance which resulted in it being denied. I had to pay for an additional doctor visit to have orders resent over. This time they provided me with false & inaccurate information stating my claim was still denied because I did not meet my deductible & because an ultrasound or xray had not been done in 30 days. I followed up with my insurance company signature as they did not sound right. I was informed that the clinic did not reset me the claim so it had not been denied a second claim & my deductible has nothing to do with it. It was denied because the ****** failed to submit my clinical etc showing my history of issues & prior procedures etc and they would need to provide that so claim can be approved. I called my doctor & the envision ****** to explain what I was told & to find out who was dropping the ball. My doctor ****** had personally walked over & delivered my doctor clinical but envision failed to do their part. I ended up having to escalate as the manger was not in the ******. I spoke with a *** who stated she was a manager who approves the claims etc & she discovered as well that the ****** failed to resubmitt my claim or send over my doctor clinical that they had in their possession. I was informed she would resubmit it for approval. Today 1/23/24 I received a call from the envision imaging ****** apologizing for their lack of getting things done & placing me at a inconvenience and assured me that everything had been resolved I had no out of pocket expenses and asked if I was ready to schedule. I asked how soon can I come I was given today at 3:30pm and an email was sent to me confirming appointment details & no balance. It stated I had to arrive 90 min before appointment and I did. Upon my arrival once again they did not have everything resolved or notes in the account so I was informed my exams had not been approved
