      Have contacted the company at least five times and continue to get the runaround. It appears that per their records, the owner of the insurance policy is incorrect. I faxed in to their number the multipurpose service form to surrender the policy including the original life insurance policy which states the owner and the insured and the beneficiaries as well as the death certificate of the owner. The *** 1099 INT has been reported in the owners Social Security number, but in the insureds name. I have tried to explain to the call center in *************** what is wrong and to remedy this to get the policy paid out. No one seems to be able or willing to assist. *** asked for a ** corporate number to call and told that *************** call center is the only number. They are working from a script and have no idea as to what I am saying.Originally called on January 29, 2024 to request the form to surrender the policy and waited two weeks and never received. I called back again and was told I could get the form online which I did and also asked them to mail a form which took another two weeks to receive. I have correspondence that the original policy taken out in 1965 from ******** Annuity Life Insurance Company was purchased by ******** National Life in 1987. *** gathered that there was a clerical error made at some point that changed the owner name of policy to insureds name, but kept social security number of Owner.In summary, this policy should be paid to me (the spouse) as beneficiary , because the owner was my husband and his social security number has been claiming the interest all these years since he IS the owner per original policy documents. Beneficiaries are parents of the insured share and share alike, or the survivor of them.

      Mid-west Life Insurance is sending payment notices for $0.83 without explanation. Called twice and was told I would get a letter in the mail explaining the charge. First call was 2/2/24 and Last call was today 2/21/23 with a person named ***** who hung up on me. I have been told there is no other way to contact the company. I have no way to resolve. Their payment notice states the life insurance policy will be terminated unless payment is made.

      I am rejecting this response because:  I am the policy holder and want an explanation of charges to be sent to me. I have received additional charges in the interim since filing the complaint. 
      This is a simple request that is getting out of hand. Please  respond to the issue with specifics lime any reasonable business would.

      This policy was started back in ****. I was supposed to pay it off within 20 years. I am still paying for it. I have no online access and no monthly or yearly printout. They refuse to send any copies of the policy or payment history. They refuse to allow me to add a beneficiary.

      After initially contacting Mid-West National Life Insurance Company of Tennessee on October 6, 2023, they have not paid out the life insurance funds for my father's policy. It took 8 weeks before they would even send me the claims form. Once I received the claims form, I submitted all forms, death certificates, certification of trust agreement, along with 1st and 2nd Amendments to the Certification of Trust agreement on November 21, 2023. I received a letter from them dated Dec. 15, 2023, stating that if the will was to be probated, they needed a copy of the court appointment naming the legal representative. The will is not to be probated. I have sent them the full Certification of Trust naming 2 beneficiaries. I am the sole successor trustee of the estate and the Certification of Trust states that. I have never worked with a company that is so infuriating.

      I am rejecting this response because:I have been told numerous times (since the October 11, 2023) by Mid-West National Insturance Company that they are checking into the matter. As of today, I have called them 13 times, have provided claim forms stating I am the successor trustee, and my sister and I are the beneficiaries to the trust. We have both sent in claims forms even though I am the successor trustee of the trust and my claim form should be enough. I have proivided my father's and my mother's death certificates, certificate of trust, the first amendment to the trust, the 2nd amendment to the trust. I am waiting to receive the funds that are owed to my father's trust. I am the successor trustee of the estate and will not quit until the funds are paid.

      I am rejecting this response because Mid-West National Life Insurance Company of Tennessee again is giving the run-around. They always wait 10 days from the time of my contacting them by written document through the ** *********** before they will respond to my request. They will not give me their email address to contact them, and they will not allow me to talk to someone in the ***************** or to their Supervisor. The other life insurance companies that I have worked with have been very efficient in paying out the policy funds within 2 -4 weeks of my first contact. This has been going on with Mid-West National Life Insurance Company of Tennessee since October 11, 2023. Each time, it takes 3 weeks to get a response back from them from the time that I mail them the nformation that they request. They make it as hard as possible to get in touch with them and my policy is drawing interest money on their behalf.

      I am the successor trustee of my father's ****** BT0004086. Mid-West National Life Insurance Company of Tennessee has the Certification of Trust with the 1st Amendment and the 2nd Amendment to the Living Trust Agreement dated June 21, 1999.

      This email serves as written documentation to Mid-West National Life Insurance Company of Tennessee, giving the Better Business Bureau and their employees, the right to information concerning ****** BT0004086. I have also attached the Certification of Trust with the 1st Amendment and the 2nd Amendment to the Living Trust Agreement, showing that my father and mother are deceased and I am the sole successor trustee of the Living Trust agreement.

      This is a horrible company to deal with. Based in Tennessee within *******, you can only speak with off-shore/*********** Representatives. They can NOT think outside of the box and make common sense decisions. Supervisors are NEVER available and NEVER call back. **********Law (out-of-state) was Owner of Husband's policy. He died. For 6 months we jumped thru ALL the red tape. Provided Death Certificate. Proof of Probate. Testamentary Letter from court. Signed, notarized ($17), fax this, mail that. Ownership was changed. Had to request proof 4 times. Then changed Benificary. Jumped thru hoops. Completed their specific paperwork. Fax this, mail that. Received proof of change AFTER 3 requests. Now after all changes have been completed, we would like a copy of the policy (original lost from deceased Father). Representative states, must submit written request and pay $25. WTF!? Dealt with Globe Life and Liberty National and it was so easy and FREE.I would NOT EVER recommend this company for anything. Beware of the headache you will have to deal with. I can only imagine what CF will accur when submitting a death claim.NO. NO. NO.

      I am rejecting this response because:

      I want confirmation that I will receive my FREE "certified copy" of my policy with ALL updated recent amendments.  I want a "specific" contact persons name and phone number to call who will lead this request.  I am the "NEW" Owner of the policy that was transferred to me  from a "DEAD PERSONS" estate and I should not be made to have to endure this.  I complied with all the changes of the ownership.  I complied with all the changes in beneficiaries.  I complied with completing all the required forms.  I faxed.  I mailed.  I called. I followed up.  YOU know exactly who I am and what my end goal was because YOU told me the steps "IN ORDER" that I needed to accomplish my goal.  Now you are making what was a seamless process with Globe Life and Liberty National a living ****.  I should NOT be penalized for wanting a "copy" of "MY" policy.  I can't get it from the DEAD.

      WHO is she???? How does she being from "Wilton Re" have anything to do with "Mid-West National Life Insurance Company of Tennessee"?   Her email is fully of sentence structure errors and grammatical errors.  Not once does she ever mention "Mid-West National Life Insurance Company of Tennessee" nor does she address me by "*****************".  How am I supposed to know that is not a SCAM?????  So very much unprofessional.

      Bottom line - I want a contact name and phone number from "Mid-West National Life Insurance Company of Tennessee" to call a get what is MINE.

      This is the absolute worst company ever to deal with. my mom passed and had a insurance policy from my understanding for long time with them, and they reached out to me in reference to being the beneficiary of the policy ( I received two separate letters about this), when I finally did reach out to them for info about this letter NO ONE could answer any of my questions, and they have these foreign people working for them (who, are not in my opinion) knowledgeable about death benefit/life insurance fluent). they are wanting me to continue a policy and continuing giving them money, but no one is able to tell me the type of policy or furthermore any info on this policy and all this was noted while I was sitting in my attorney's office at that time and making the phone call. I hate and I am very upset that my mom chose to deal with a company who is so uneducated on family matters with policies and shows no compassion/ or any concerns, for deceased policy holders family members. This company needs to be reported to the senate and others because they are not compassionate or concerned about helping families find out the correct answers to the things they need.

      Received letter at my home address from Mid-West National Life Insurance Company of Tennessee, *********, **. giving a policy number and asking me to fill out a Request for Taxpayer ID Number and Certification form. When I called to inquire I was told that I owned an annuity policy with that company. I do not own any kind of insurance policy with this company nor do I own any annuity policy with any insurance company. I believe this company has acquired personal information of mine without my knowledge or consent. I am taking steps to prevent the illegal use of my personal information.
