My son was send on 10/06/2021. I submitted a request for his medical record on 10/14/2021 via email as stated on the website. I have not heard or receive a response. I also faxed the form twice since then. I have also sent follow up emails on 10/20/21, 11/1/21, 11/8/21, 11/11/21, 11/22/21, 12/14/21 and 12/28/21. Last week I called the office in **** and they said they would email the **** for me and call their manager. After a few days they also stated they could not reach the department. I need my sons records to file a insurance claim. The website and the voicemail for the medical record department states it can take up to 30 days and it has been way past that. I also call weekly to the medical records **** and leave voicemails as well. I can forward you the emails if you would like.Business response

I came onboard recently as Clinic Manager and i have looked into the situation. I personally had my Front Desk supervisor fax a request to our Medical Records to send Ms. ******** the records she requested.
I apologize for the lack of follow-up. You mat give ******************** my work cell number ************, and to follow up with me directly if she has not received her request by Wednesday of next week.
