Medical Services rendered 6-4-2021 Medical claim sent 07-29-2021 Acceptable reimbursement made 8-03-2021 Ref# ************* Check# *********** -Case closed in A/R ... Sept 16, 2022- Cotiviti sends refund request with this detail- "Cigna plan #******* is primary fo r this patient effective 02/01/21 because the plan that covers a patient as a subscriber is primary to a plan that covers the patient as a dependent. COB type is standard." Research determines: Patient reports he has no other insurance at the time of the surgery, Cigna representative confirms that the other Cigna plan became inactive 12/31/20 Call ref : ******* Appeal sent once reminded of this outstanding overpayment, multiple calls to Cotiviti seeking retraction letter. No money requested, we only seek retraction of this overpayment so we can close the case. Please email with questions.
