            Hello,I was seen for a consult at Lake Barcroft Dental Group on 10/04/2021 for tooth pain. On 10/14/2021 I was seen again for a tooth extraction. I paid $100 on 10/04/2021 and $354 on 10/14/2021. I was told by the dental office I had to pay for all services rendered up front and any insurance payments would be sent to me. According to the dental office, they submitted claims to my dental insurance company for all charges on both days. I received an Explanation of Benefits from my dental insurance for 10/04/2021 indicating they paid $77. My insurance provider has no record of a claim for 10/14/2021. I called Lake Barcroft Dental Group on 11/9/2021 and they assured me they submitted the claims; it was automatic. To investigate, I requested an itemized list of all charges and payments from Lake Barcroft Dental Group. I was given a copy of a treatment plan that only included estimated charges and I was told that was all they could provide. I think Lake Barcroft Dental Group has kept payments from my insurance instead of sending the payments to me.
