      Sent cancelation request for coverage in April of 2021 only way to cancel is through fax. I have been paying $40 a month since then without my knowledge and thought the coverage was canceled.

      Per *********'s request, we have terminated his life insurance coverage and immediately ceased his electronic funds transfer--he will receive a termination notice from MBA via USPS mail.

      We do not show a record of contact from ****************** or previous requests to terminate coverage, which we would have been happy to perform anytime.  MBA members can communicate with us via phone, email, or their on-line accounts.  Were thankful for **. *******’s service and stand ready to serve him for any future needs.

      Applied for Life Insurance 5/28/2021. Effective Date of Policy 7/01/2021.Called 7/14/2021 as no policy received, told to wait it was being sent out. Called 8/3/2021 as no policy had arrived. Told that a supervisor that would be contacted immediately to send out the policy. Called 8/17/2021 as no policy had arrived. Assured the policy was being sent out. On 9/02/2021 no policy has arrived but a envelope containing Official Proxy Ballot 2021 arrived. Have paid over $142.74 in premiums; $47.58 monthly auto pay from my **** account. Policy #********

      MBAregrets that USPS was unable to deliver *** ***********'s insurance certificates.  MBA sends hundreds of thousands of parcels every year successfully, to include an envelope sized letter *** ************* received, so we're perplexed why the larger certificate parcel never made it to him.  We have spoken to **** ************** and while he declined a third, overnight/registered mail attempt, we have terminated his plan at his request and are reimbursing him all monies submitted.
