Initial and only contact with Empower Neuropathy occurred on 4/27/22. A description of program services were provided. l signed an application for a 90-Day Reversing Neuropathy Program, providing home care therapy. Unfortunately, I signed an application form which also authorized ******* *******, a loan company and ******** ***** **** ********** **, Lincoln, NE *****, a load servicer. Although I never received treatment from Empower Neuropathy program payment notices are being sent to my home. Requesting payment of the program principle amount of $7,480.00 and total cost $9,758.65. I have received financial records that have listed me as a Borrower because I unknowingly signed a loan agreement at my initial meeting with Empower Neuropathy. On 5/12/22, a Treatment Cancellation Report was faxed to Empower Neuropathy, a letter was also sent to ******* *******, the loan program sponsor and coordinator. I attempted several telephone contacts with Empower Neuropathy. No answers or responses were received. On or about 518/22, an undated letter was received from Dr. Callan L********** at Empower Neuropathy. The letter indicated that his office tried multiple times to to call me with no answer or return call. The letter also indicated that a refund is not possible as the equipment and protocols received are non-refundable. The letter also indicated that this was explained ini depth and written very carefully in the document I signed. Not exactly true.
