    On March 6, 2024, I paid $135.00 to this dentistry after an unfortunate phone call with a representative by the name of ********* who didn't provide substantiation for the invoice I received and threatened to send me to collections if I didn't comply. To date, I still do not quite understand why the visit charged for (on January 11th) wasn't covered by the dentistry's warranty policy on dental crowns, when the ~6 other visits in the same time period were covered (see attachments). My desired resolution is either or both of: (1) an explanation of charges and (2) a refund if a substantiation can't be provided. Background is below:On 01/08/2024, my root canal-ed ***** was prepped by the dentistry for a third crown after the first two (also done by this dentistry) had issues with bite, retention, or both. Follow-up visits regarding this *****'s crown were never charged to me in accordance with the dentistry's warranty policy, as was my understanding. Dental crowns need to be made at specialized facilities, and so temporary crowns are used between the date of the preparation of the ***** and the date of the delivery of the final crown. On 01/08/2024, I was given a choice on whether to use the outgoing crown as the temporary or have a new one made, and I chose to go with the former. The outgoing crown did not retain on the ***** now that it was prepped for a different crown, and so I came back in on 01/11/2024 to have a proper temporary crown made. (This is the visit that resulted in the disputed charge.) This temporary crown in question also did not retain and led to great discomfort and bleeding, and so I returned on 01/15/2024 to get the temporary crown re-shaped. The permanent crown was delivered on 02/07/2024. The warranty policy is for 5 years from the original crown delivery, which was in fall of 2022. The patient must adhere to all callback appointments (i.e., 6-month checkups) and receive fluoride at each visit, which I did.
