      I am writing to submit evidence regarding my claim against my orthodontist. He involved malpractice in my treatment.He told me in January of this year that my treatment was finished, but it was clear to me at that time that there was a serious problem with my bite, which was causing me difficulty chewing. In addition, he did unnecessary IPR on one of my healthy front teeth without my prior notice or consent during the treatment process, leading my dentist to take an X-ray saying that my front tooth may be damaged and may require root canal treatment in the future. So I refused to sign the consent form to end treatment and asked him to continue to adjust my bite, but he insisted that my dental treatment was over.??In the next few months, I suffered from physical and mental pain because of my inappropriate biting of long periods, I couldn't enjoy food, and I was afraid to even laugh and take pictures in case someone noticed my asymmetrical teeth.??I started to consult other orthodontists, and they all said after looking at my teeth that I had occlusal problems and that it would take 6 to 12 months to fix my issue, and the cost of treatment would be between 4k and 6k. I asked my orthodontist again to continue my unfinished treatment, but he refused. I obviously couldn't afford to re-treat cost, so I asked my orthodontist for a refund so that I can get retreated with another doctor.??Attached is part of the report and quotation of my consultation with other orthodontists currently. Some orthodontists are reluctant to write a report for patients whose treatment has not been completed (the attachments on my teeth have not been removed due to dispute on the treatment results). My dentist advised me to continue to consult the doctor about my *** problem, as I would hear my bones clucking while eating and have difficulty opening my mouth, and I would keep the *** doctor updated on the evidence. Because when I am trying to open my mouth, I always hear a clucking sound.

      Patient presented with crossbite and Class II malocclusion, which was successfully treated. At the patient's request, the treatment plan was modified multiple times. This led to increased treatment time. The patient disengaged from the treatment process at the completion and ceased communication following the completion of treatment. Based on the terms of her contract, the patient was was refunded 10% of the care for the removal of the attachments and clear retainers. In response to a grievance filed, the patient's treatment plan was reviewed by a team at Delta Dental where a panel of experts (Delta Dental Consultant, Appeal Specialist and Quality of Care Coordinator) who determined that the care was rendered adequately and as planned and supported the refunding of the final stage of treatment; removal of attachments.

      I asked **** for the record of my history, so that I can better treatment my teeth and describe to third party but he didn't response to me.

      I am rejecting this response because: First of all, I didn't want to change my treatment plan, my only request was to do it in a less invasive way (which I discussed with other orthodontists and thought was perfectly feasible), and rooz didn't respect my thoughts and concerns. Instead, he took a radical approach, and without discussing it with me beforehand, Completely unnecessary IPR to my teeth, causing damage. Secondly, the occlusion of my teeth is completely asymmetrical, requiring an additional 6 months to a year of treatment, and based on the assessment that this is a completely treatable problem, I do not believe that a dentist would refuse to treat a patient and refuse a refund. Third, I asked **** to give me the previous file, because the dentist in charge of follow-up treatment needs to know my medical history, and **** refused my request (no reply). As a dentist, he had no right to refuse to send me patient data. I think he has been improper and derelict in the whole process
