      Ive purchased private health insurance with advantCare 3 first health network PPO. a representative name *** his badge #***** His direct number is ************ sold me health insurance. I recently injured my ankle and needed health insurance from March 1st- April 1st until my current employer provides health insurance April 1st. After inquiring about if my specialist will accept this insurance *** stated first health insurance is accepted by all providers and specialists. I inquired if theres any issues with insurance can I be refunded 100% I was told yes. When I set an appointment with my specialist orthopedic they stated they do not accept the insurance. I called *** back a few times left numerous voicemails and text and he never returned my call. I then contacted main number line and spoke with a 3 different representatives overtime who stated they will help me and nominate the specialist and return my call. I also inquired will insurance pay my ER bill prior to purchasing the insurance I was told by a representative that yes they will pay 50% of bill if submitted. Today 2/28/23 I found it that was misinformation as well they will not pay any back bill before insurance was purchased. I was told today 2/28/23 I will not be refunded my money because it was the day before my insurance takes effect. However last representative 7 days before on a recorded line stated she will give me a call back in 3 days to inform me of update if my specialist has nominated me. If not she will refund my money. I never received a call back. I paid almost $300 for health insurance and I have no provider have not seen a specialist because no one accepts health insurance within radius and I cannot be refunded my money and I was misinformed and lied to. This company has my money and I never saw a specialist.

      ON JULY 1ST 2022 I CALLED INQUIRING ABOUT GETTING MYSELF SOME HEALTH INSURANCE AND I SIGNED UP WITH FIRST HEALTH NETWORK IN WHICH I AGREED TO PAY ***** TO SIGN UP AND THEN I AGREED FOR MY DEBT CARD TO BE DEBTED ON JULY 15TH 2022 FOR THE REMAINDER OF THE ****** FOR THE BALANCE IN WHICH I OWNED FOR SIGNING UP FOR MY HEALTH INSURANCE. I'M REALLY UPSET BEACUSE I HAD AUTHORIZE THIS COMPANY TO TAKE MY PAYMENT THRU AUTOPAY THE 15TH OF EVERY MONTH BUT ON AUGUST 10TH I RECEIVE A FRAUD ALERT FROM MY **** STATING THAT THIS COMPANY TOOK IT UPON THEMSELVES TO TRY TO TKE MY PAYMENT 5 DAYS IN ADVANCE WHICH IS A NO-NO. I CALLED MY **** CAUSE I DIDNT AUTHORIZE THIS TRANSACTION AND THE **** TOLD ME THEY WILL ISSUE ME A NEW CARD SO THE COMPANY WOULD NOT TRY TO TAKE ANYMORE PAYMENTS. THEN I CALLED FIRST HEALTH NETWORK TO CANCEL THE INSURANCE AUGUST 10TH 2022 DUE TO ALL THE ISSUES I HAD DEALING WITH THEM FROM THE START. I WAS TOLD THAT MY HEALTH INSURANCE WOULD GO INTO EFFECT JULY 15TH 2022 AND I CALLED AND CANCELLED ON AUGUST 10TH 2022 AND I WAS TOLD THAT SINCE I HAVEN'T HAD THE INSURANCE FOR 30 DAYS I WAS INTITLED TO A REFUND AND IT WOULD TAKE UP TO 2 WEEKS TO RECEIVE IT AND IT HAS BEEN OVER 2 WEEKS AND NO REFUND IN THE MAIL YET. I CALLED ************ AND 215=987-2998 TRYING TO RESOLVE THIS MATTER AND I KEEP GETTING THE RUN AROUND ABOUT MY REFUND

      First Choice Health Network, Inc. of *******, ********** is in receipt of the Complaint filed by ***********************.  First Choice Health Network, Inc. is NOT an insurance company.  First Choice Health is a Third Party Administrator and a PPO Network.  First Choice Health does not sell individual insurance policies.  There are various corporations across the country that use "First Choice Health" within their corporate name.  First Choice Health primarily does business in the Pacific Northwest Region.  I have reviewed all of our records and do not have any record of a *********************** having called.

      I am sorry that we could not be of further assistance.

      MY MEMBER NUMBER IS ********* WITH ************************ IN WHICH WAS CANCELLED ON AUGUST 14TH 2022 AND I WAS TOLD I WAS DUE A REFUND *** TO NOT HAVING THE HEALTH INSURANCE FOR 30 DAYS AND THE ONLY WAY FOR ME TO GET MY REFUND IS IF I GIVE THEM MY DEBT CARD BECAUSE THEY DO NOT ISSUE CHECKS MY REGULAR MAIL. THE ***** NUMBER TO THE ENROLLMENT CENTER IS ************ AND THE *********************** NUMBER IS ************. MY HEALTH CARE PLAN WAS WITH FIRST HEALTH PPO OUT OF ************

      First Choice Health Network, Inc. in **************** does not operate as an insurance company.  First Choice Health does not offer any insurance products, including individual health insurance.  First Choice Health is a Preferred ********************* and a Third Party Administrator for large self-funded health plans only.  First Choice Health does not collect information for initiating a bank draft over the phone.  First Choice Health does not have either of the phone numbers ************** or  **************.  which is are ************ area codes.  First Choice Health operates primarily in the Pacific Northwest.  FCH operates out of *******, **********.  There are many organizations throughout ***************** that have similar words in the title of their company.   It must have been a different organization, not First Choice Health Network, Inc. of *******, **********.  I am sorry that I am unable to assist you further.
