      The company I work for goes through Asuris insurance for us. I need a jaw surgery to repair it, and I went through them. They originally denied it, but the surgeon filed an appeal which they *****ed. So I get excited. Finally no more pain, no more problems swallowing or trying to chew my food. Then the surgeon tells me they have to put a splint in during the surgery. So we file with the insurance and they deny so we appeal and they ***** it saying it was deemed medically necessary. So we set up the surgery. A couple days before I'm supposed to go in centeral washington oral and facial surgery calls me and saying there's an exclusion and this surgery isn't covered, yet they approved it. Centeral washington oral and facial surgery spent a lot of time and so did I. This is so wrong. They can't approve, approve, approve for months and then pull the plug at the last second. If there was an exclusion they knew about it the whole time and never said anything! So I'm reporting them to you, I'm calling the stste attorney generals office to have them investigated. They said I have to get a tmj diagnosis for the surgery to be approved. I said no you already approved it. I spent money at the dentist to get my existing teeth stabilized for this surgery. So this company has lied, and cost me a couple thousand dollars. I want them punished, I want the surgery approved which they already have, and I want compensation for the c*** they've put me through.

      Dont let your company use this form if they have employees not located in *******. Nobody can find them and their coverage is terrible
