      My first contact with NW Spine and Pain was on 9/8/23, when I was evaluated and accepted as a patient by *******************************, MD at **************************************** that time I signed a "contract" guiding what I could and couldn't do when being prescribed the Buspar patch. One item in the contract was that I would not use diazepam. I explained to ****************** that my panic attacks are not common, but they are bad and that diazepam is what I take for them. I asked what to do because by signing the contract I was agreeing to not take them, but in the midst of a panic attack I need to because at that time, it's an emergency.He didn't respond except with a shrug. He offered no guidance, so I signed the contract. After a couple of visits with ******************, I moved to the north office to see ***********************, ** at ***********************************************************************************************. At our first, and subsequent visits, I again mentioned that when a panic attack happens I need to take the diazepam. I asked what else I could do. She said she didn't know, but she understood that I'd be taking it if absolutely necessary. She may or may not have said so in words, but she was very understanding and it seemed clear to me that wouldn't be an issue.While *********************** was out on maternity leave, I saw ***********************, ARNP,NPCk at the ********************. It was at my appointment on 5/01/24 that he said I am disenrolled because I tested positive for diazepam, which I had used within a few days of that drug screen test I was asked to take. He refused to answer my questions (I had to ask him several times to clarify that he was disenrolling me), didn't look at me, and left quickly.In the two weeks since, I have left several messages with their office manager asking for information and help for what to do next, since *********************** only prescribed one month of the Buspar patch and it takes a long time to find alternative providers, she has not returned any of my calls.

       After going through my medical record, I want to add three documents. The first is regarding the discharge letter ***********************, ARNP,NPC sent to my physician at ****. It contains both false and misleading information and it makes me look as though I did something wrong, when in fact I had been clear, open, and honest with each of the three providers I had seen at NWSPM about my occasional and rare use of diazepam for panic attacks. I had received approval from my provider there (who was out on maternity leave, thus why I was seeing *********************** at this time), and she had indicated this in her medical notes for my appointments with her. I include those passages in the attached document named "Disharge note about lack of trust *********************** on 1-24".

      The second attached document, named: "Notes re- diazepam discussed and approved" has the parts of my medical records where my provider, ***********************, had recorded her knowledge and approval of my occasional and rare use of diazepam for panic attack.

      The third attached document regards *************************** medical assistant, *********************' notes on the call log for my call on 4/02/2024 08:46 AM regarding a prescription that had not been sent, or at least not received, as promised after my appointment on 3/26/24. She describes our conversation using both false and misleading language and descriptions. In the document I have her notes and my response to them.
