I visited the emergency ********** of St ****** due to an ankle injury.Despite my frustrations about my entire treatment and conversations with the doctor happening in the hallway with 20 other people who could easily hear all the medical information I discussed with the doctor, to my disappointment that was just the beginning of the worst. I first received a bill from Empower Physicians where it was quite clear nothing had been submitted to my insurance . After discussing with a few trusted individuals I called in to provide my insurance again. During this time I was receiving threatening calls multiple times per day that I would be turned over to collections. I paid one small payment of roughly $80 to get them off my back before I realized the insurance issue. My insurance has since processed the claim and my amount owed was only $19.89. I have NEVER received the difference of that money back. To top it off I am also getting bills for the same emergency services from St. ****** hospital for just over $900. *** called to ask why Im getting bills from two different entities for the same service and neither company says they know anything about the other. I have logged into the account and set up payment plans TWICE for this bill and paid their initial down payment twice and still getting calls where the reps are saying there is no standing arrangements and I log back in and it says the same thing. They have withdrawn the money both times but never sent receipt of payment and it feels like they are purposely trying to nudge me into collections. I feel like Im being charged for the same service twice and nobody can give me answers why.

On 6/10/2023 I went to the emergency room. sometime around the first week of July I received a bill from this company that they had not put thru my insurance. I called them on 7/12/2023 and spoke with ******, she informed to ignore the bill, that they just sent it to my insurance company on 7/10/23. I now received a text demanding payment or it will be turned over to collections. They still have not sent it to my medical insurance and are demanding I pay as if I do not have insurance. I want this taken out of collections and submitted to my insurance. My account number for this bill is **********. I have checked with my medical insurance and they never received a claim from this business.

I have been billed final notices without receiving any notifications from this source. I called for documentation regarding the services provided and none were provided. It was explained that initial notices were sent via text message. They threatened my account being sent to a collection agency without giving any assistance or documentation to support the reasoning. Additionally, the emergency physician is not known to the facility providing the claimed services.

I visited the Emergency Department at Dignity Health on 8/21/21. I continue to receive a bill in the amount of $1282.00. I have contacted my insurance (BCBSofAZ through Medica) and they have never received a claim and nor do I have an EOB to show for it. Their call line is down. You are unable to speak with anyone over the phone via **************. Anytime you chat with someone it never gets resolved. Historically, I have a very hard time getting my insurance submitted correctly. I've tried to give them the provider services number ************ for my insurance to allow them to handle it, but it continues to be unsettled.

The failed to **** my insurance company charged me sent it to collections and had a law firm *******II LLC reach out to me to threaten to take me to small claims court. I faxed over multiple times my health insurance information and have records of me trying to call email them and no response. I received a notice from a BME Services Debit Collector on February 3rd, 2022 for a visit to the Emergency Room on August 21st 2021. The collection amount of $883.00 is owed to EMPOWER EMERGENCY PHYSICIANS GROUP, because they submitted my secondary ********* of ******* and not my primary ********* of ******** BCBS. Ive attempted several calls and voicemails to this scam of a company. I contacted ******* whom stated they denied the claim because ******** wasnt billed first. I contacted ******** whom stated they have no record of receiving the Physician Claim from EMPOWER EMERGENCY PHYSICIANS GROUP but that they did receive the Facilities Claim and successfully processed the Facilities Claim. ******** also gave me an EOB which confirmed this $883.00 **** would be covered in full if EMPOWER EMERGENCY PHYSICIANS GROUP would simply file the claim to BCBS as theyre supposed to. I also contacted the billing department of the ** who confirmed they have all my correct ********* information on file, they advised me getting a lawyer for attempted ********* fraud as they stated this isnt the first issue theyve heard of with EMPOWER EMERGENCY PHYSICIANS GROUP intentional not billing ********* companies despite having all the accurate information.
