Visited AED Urgent Care (formerly known as AFC Urgent Care) on 06/29/2023, and upon entering was told both Healthcare Highways (primary) and Tricare Prime Remote (secondary) were in-network & covered for services/care. As the pt, I fully expected the combination of in-network services to pickup the entire balance; however, on 11/04/2024 I rec'd in the mail a statement/bill for $413 and stating ************ Information on File; note: this bill was printed & mailed +16 months after the date of service. After immediately calling and finally reaching a pt services ***resentative, who confirmed both primary & secondary insurance info on file, was told a billing *** would call me back. Never did, but during that time AED Urgent Care did attempt to file a claim with Tricare, which was denied since they failed to first submit to primary insurance & provide a copy of the *** with the claim to Tricare/secondary. Rec'd a 2nd/follow-up bill on 03/14/2025, called AED's service line, and "Kayr" told me a billing *** would call me back within 24 hours. That never happened, and thus after ***eated attempts on 03/24-26 I reached a *** again who finally advised the claim would be resubmitted to both insurances.California Labor Code section ****** states, "For services rendered, or inpatient discharges, on or after January 1, 2017, there is a statutory requirement that medical bills be submitted within 12 months of the date of service, or within 12 months of the date of discharge for an inpatient bill".Following my call with billing ***resentative ****** I found online multiple ***utable new sources confirming AFC Urgent Care's fraudulent billing practices that are under scrutiny and in the California courts over similar bills following extensive delayed timing post-date of service(s).Initial Complaint

This business is sending me a new bill (statement date 1/16/2025) for services that I have paid two years ago (in year 2023). In their charge, they included a suspicious "Insurance Error", which is the amount I owe. This "Insurance Error" totally makes no sense to me. Back then, due to the pandemic worldwide, COVID tests were FREE. We went to AFC Urgent Care in ****** that had their banner say FREE COVID TESTS to get tested and provided our insurance. The doctor and staff also confirmed that these tests are free.Almost 4 years later we received their bill demanding THOUSANDS of dollars, or else NO ACTION WILL GIVE US AUTOMATIC PERMISSION TO FILE LAWSUIT TO RECOVER OUR BILLS PLUS CHARGES TO FILE THE CASESWe tried to reach out but there is no phone number listed on the bill.This SCAM must be reported and stopped ASAP.They are apparently doing this with everyone who got the free covid tests from them. I went to get a Covid test back in 2021 to this clinic along with others, its said it was a free Covid test, but it did say free testing and was in front of clinic as well, and was asked to give insurance information as i did, and now years later they are sending me a bill of $2700. for testing.Initial Complaint

On 12/23/2020, I and two of my family members, went to AFC Urgent Care for Covid testing. This was during the pandemic, and we could not get an appointment anywhere in our insurance network. AFC Urgent Care had a huge sign stating they were offering FREE Covid testing. Also, I had check** with my insurance, and they had advis** me that any services for Covid testing would be cover**. *** took our insurance information, and did not ask us for any payment. On 11/25/2024, I receiv** three invoices requesting payment for "m**ical services" dat** 12/23/2020 for the total of $1146.00. The invoice has AFC Urgent Care's name on the top with their logo, but the payment stub is address** to *************. They are one and the same company. First of all, this is inappropriate, as during the pandemic from March 2020 to January 2022, and illegal as I research** the Senate Bill 510. It states that all m**ical establishments/providers providing Covid relat** services, even if out of network, were to bill the patients' insurance carrier and no balance was to be bill** to the patient. 2nd of all, trying to claim money for services done FOUR years prior comes from gre** and is totally ridiculous. 3rd of all, I contact** my insurance carrier right after receiving these invoices, and they had inform** the the bill** amount was $922 for each claim and the paid amount was $540 for each claim. The invoice I receiv** from ************** states my insurance carrier only paid $176.00. That is a LIE. Saloojas/AFC Urgent care is trying to scam money from innocent patients, from a pandemic and F**eral EMERGENCY, and I will NOT PAY. They should be asham**. The invoice threatens to send the case to small claims court or send to collections if not paid. I mean can you get any SCAMMIER! I want these invoices zero-** out. I OWE NOTHING.Initial Complaint

Purpose: I am submitting a complaint against *** Urgent Care for incorrect and potentially fraudulent billing practices. Despite being informed that they were in-network with ***************** PPO, the billing records do not reflect insurance adjustments, and I am being charged inappropriately.Amount: The billing statement shows charges of $385 for the visit on 05/20/2024 and $383 for the visit on 09/08/2023. Despite a patient payment deduction of -$50 listed for the 05/20/2024 visit, the claim still wrongfully charges $385. The total amount due, according to the statement, is $768.Request: I am asking *** Urgent Care to revoke the charges totaling $768 and address the discrepancies, as well as provide compensation for the inconvenience and distress caused by their billing errors and misleading actions. Additionally, they need to correct the insurance filing and ensure all claims are properly processed according to network agreements.Full Context: I visited the *** Urgent *********** twice, on 09/08/2023 and on 05/20/2024. Both times, I was explicitly informed that they were in-network with my insurance, *********************, and I paid the confirmed copay.I later contacted my insurance company, and we made multiple unsuccessful attempts to reach the urgent care's billing department. The insurance company informed me that the claim for the 05/20/2024 visit is incomplete and cannot be processed. They emphasized that I should not be charged for it.Moreover, the insurance company stated that I am not liable for the charges from the 09/08/2023 visit. The doctor I saw that day, *** ***** However, the urgent care center submitted the claim using the name of a different doctor, *** *****, who is not in-network. Visited an in-network urgent care. Saw it as in-network urgent care at insurer site. I was recommended a covid prescription but I did test at home. Urgent care filed case as out of network to my insurance and sent a bill. I took it to insurance and they agreed their is an issue but Urgent care is unwilling to file claim as in-network and adamant to send me bill as out-of-network patient. I am writing to formally report what I believe to be a case of serious healthcare fraud and harassment involving AFC Urgent Care of ******. This situation has become unbearable and is now affecting my well-being and peace of mind.I received a bill recently for $3,200, allegedly for a COVID-19 test I took in 2021. This claim is shocking, as COVID-19 testing was widely advertised as free during that time. Since this initial bill, I have been inundated with a flood of harassing emails, calls, and threatening notices from collections, demanding immediate payment. This behavior feels like nothing short of blackmail and psychological harassment.I strongly suspect that AFC Urgent Care may be targeting patients who underwent COVID-19 testing under the assumption it was free, only to retroactively bill them for outrageous amounts, with no transparency or legal grounds for these fees. To make matters worse, there are multiple victims like myself experiencing this exact pattern, leading me to believe that this is part of a widespread fraudulent scheme designed to take advantage of innocent individuals.The level of harassment is extreme: I am receiving calls and emails weekly, if not daily, and the tone has escalated to threats of legal action and impacts on my credit if I fail to comply. This goes beyond a simple billing disputeit feels like extortion. The stress and anxiety of being constantly hounded over this illegitimate bill are significant, and I am deeply concerned that this pattern may continue to harm other innocent individuals.I urge you to investigate this matter with the utmost seriousness. The health care sector should be held accountable, especially during times like the COVID-19 pandemic, when ************* was paramount.
