      I was in for a visit when one of the employees by the name of **** tried to make a pass on me he also made a sexual comment which left me very uncomfortable . At that point I was too nervous to say something . I am not homophobic but this was highly unprofessional and uncalled for. I do want I be anonymous and do not want my name published . If this is happening to me Im sure its happening to other patients . No one should have to feel this way when you feeling ill.

      PA ********************* practicing at Carbon Health North Hollywood engaged in physical abuse involving assault and battery of a minor child patient. The 2.5-year-old child was brought in for a Covid test. At the time of the visit, the child was not exhibiting any Covid symptoms. Against the mother's continuous protests and requests to stop, and the child's pain, discomfort, and crying therefrom, PA ***** inflicted physical force to subdue the child in order to run unnecessary diagnostics, causing pain and extreme emotional distress to him and his mother. Among other things, and after it had been confirmed that there were no signs of illness, PA ***** force opened the child's mouth against his and mother's will using a stick and pressing forcefully on his tongue, causing pain and extreme distress, all despite having been warned that the child has severe reflux and could vomit. PA ***** also pried the child's ears open causing the child additional pain and stress. Despite the mother's continued protests and the child's pain and resulting emotional response, PA ***** then used her physical force to further subdue the child and to force tape him to a diagnostic tool. As part of this abuse, PA ***** was twisting the child's finger and inflicting pain to force him to comply. Throughout PA ******* lengthy abuse, PA ***** continuously refused performing the requested Covid test and conditioned it on the above conduct. When PA ***** finally completed the entirely unnecessary and horrendous actions above, she rejected the mother's attempt to hold her child so that a Covid test could be performed. Instead, PA ***** used both her arms and full body force to pin the little child's body down so he could not move and to yank his head back. Following PA ******* conduct, the child was inconsolable and was gagging. At the end of the torturous abuse, PA ***** stated she was going to run unauthorized tests, clearly to elicit a higher insurance payout and attempted to pressure consent.

      Date of the transaction 1/26 I paid $150 for a 24hs PCR Covid test, and l have never received the results by email. I need the test results to travel They told me that I was going to receive the results before 2pm. They never arrived. I want my money back

      I need to get a rapid covid test for my youngest daughter who had not been fully vaccinated yet while we were in vacation in ********** before returning home to Hawai'i. We had taken a PCR test, but the results had yet to come in and we wanted to ensure that she would not have to quarantine for 10 days, so we found a rapid test site at the ******************** a near **************************************** ****************. I filled out the appointment request online and rushed down to the airport to give her the test at 230pm on Nov 15th as we were leaving on a flight to Hawai'i at 7am from ***. When I arrived, I was asked if I had an appointment and I said yes, gave her my daughter?s name. The girl in the front just asked for my credit card. I confirmed that the test results would be ready in 3 hours and she said yes. She told me that I could access the via the account that I set up to make the appointment. I logged into the account that evening and saw no test results, no lab results, no information whatsoever. I tried to contact via the messaging feature on the website with no response (until the next day when we were already in Hawai'i). I called and there was no answer. I contacted the company and explained that I didn?t get the results and asked what happened. They later told me that I had to go into the settings and press switch account to get to my daughter?s results. This information was given one day later and of no use to us as our flight had already left **********. I requested a refund of the test as the results were of no use to us at this point (you need the result prior to the return flight). They denied my request even after multiple times explaining that I was not given the proper information to access the results. How was I to know that I had multiple users in my account and I needed to go into the settings to access that information. Sine I was not given proper information, I should not have to pay for the $135 test.
