    ************ Is A Friendly Dentist But The Material That ************ Used For My Top Front Teeth Bridge Work Is Not of Top-Grade Quality Material Because One Area I Can't Floss It And It Still Bleeds Consistently, Little Black Specks Collect And Gets Stuck In My Top Front Teeth From Eating Different Fruits. And It Takes A Safety Pin Tip To Try And Scrape Out These Little Black Specks Off Of The Cracks Of My Front Top Teeth Bridge Work All of The Time. This Should Not Be Happening! This Top Bridge Material Can Easily Be Shaved Too, But That shouldn't be Possible Either Because. My First Original Top Bridge Teeth Work That Was Done For Me By Dentist ************** In ********, **. Back In **** Was Of Only Porcelain Top Grade Material And It Was Not Shave able. And Little Black Specks Never Got Lodged into The Cracks of These Teeth on my Top Teeth Bridge Work Then Either. But Before I Knew Of **************** Top Front Teeth Bridge Implant Work Experience Because He Was Only Advertising Dental Implants at The Time In Year 2021 When I Had Gone In To See ************ for A Bottom Implant Procedure Only, But ************ Assured Me to Trust Him to Do My Top Teeth Bridge Work Because My Original Front Top Teeth Bridge Work Had Been Done For Me Already 34 Years Ago So I Agreed To Trust ************. But Later On, I Realized ************ Used Poorer Quality Material For My Top Front Teeth Bridge Work And **************** ************* Acted Very Crooked with Me Because I Was Offered Several Discounts From Coupons, blueappledental group *************, And Even I Got One Of My Discounts From Dentist ************ Himself. But Then blueappledentalgroup ************* Treated Me Like A Child And Told Me That Those Discounts That They Offered To Me Were Only with Them So blueapple dental group ************* goes And Charges The Full Amount Of Charges To ***************** Credit Account And Never Did Honor Any Of My Discounts That Were All Given To Me. Blueappledental collects The Full Amount Of Money *********************

    Patient was presented a treatment plan for the mentioned upper 4 unit upper anterior bridge involving teeth #7-10.  When our Treatment Coordinator presented her with a treatment plan in the amount of $6,625.00 for the bridge.  At that time,the patient informed us that in order for her to move forward with the bridge,she would have to finance it through her already existing CareCredit account.  Patient requested a discount to accommodate her monthly payment amount.  We approved a professional discount of $830.00 towards her total fee of the bridge, making the total for the bridge $5,795.00.  She expressed her gratefulness for that professional discount.  She signed the treatment plan that has also been attached in our response to this complaint on 08/03/2021.  
    On 08/10/2021, the patient was in our office and had bridge #7-10 prepped and sent to the dental lab.  On that same day, we ran her CareCredit account in the amount of $5,795.00 with her approval.  She signed the CareCredit receipt that has also been attached to this response.  She returned to our office on 08/31/2021 to have the bridge delivered and was cemented with temporary cement to allow the patient to adapt to the bridge prior to cementing permanently.  On 09/08/2021, the patient returned to our office to have the bridge cemented permanently.  She did not complain once about the CareCredit or her balance that she had agreed on. 
    On 10/08/2021, we received a Billing Dispute Notification from CareCredit regarding the following signed and accepted treatment plan on 02/09/2021.  The treatment plan consisted of a total of $6,500 charge for prophylaxis, #** extraction with bone graft, **** scan,diagnostic casts, surgical guide, implant and implant crown on #**.  ***** services were rendered on 02/11/2021,04/12/2021 and 06/28/2021.  A discount of $500.00 was credited back to the patients CareCredit account on 03/15/2021 because according to the patient, she was upset because she saw that we were running a mailer coupon discount of $500.00 towards this service.  This discount was in the mail after this service was even preformed.  On 04/12/2021, we gave the patient an additional internal discount on her dental account with us for the **** scan of $450.00. 
    After the implant and crown procedure on #** was completed on 06/28/2021.  Patient continued to insist that she never received her $500.00 discount and we gave her an additional $500.00 off of her dental account on 07/14/2021.   At this point, patient had already received a total of 3 discounts:
    03/15/2021:   $500 credit back to her CareCredit account
    04/12/2021:   $450 discount off her dental account,waiving the **** Scan   
    07/14/2021:   $500 discount off her dental since she insisted that her ledger did not reflect the $500 discount (even though it was already refunded to CareCredit)
    On 07/01/2022 we received another dispute regarding the same $500.00 on her CareCredit account. 
    We have included a copy of the two CareCredit disputes, the CareCredit refund receipt, her 2 signed treatment plans, her signed CareCredit receipts for the treatment rendered as well as a copy of her ledger that reflects the mentioned above professional discounts given to the patient. 
    Our office uses high quality materials along with a reputable professional dental lab.  We also provide all of our patients with proper home care instructions for after care of our dental procedures.
    In conclusion, the patient does not have an outstanding balance in our office.  She is in good standing with us.  Any pending balances are owed to CareCredit.  Please contact CareCredit for any further concerns.
