    I had a colonoscopy on 1/16/2024 at ********************** in ******, **. Prior authorization was given by Angle Health for the procedure prior to the procedure. Since this time, the provider has sent multiple requests for payment. Angle Health keeps denying the claim and states that I am not in the system. I pay monthly premiums and have done so since January 2023. I have called multiple times and have been told that I am active (but Angle Health informs the provider otherwise). The provider and I am out of options at this point. I was told the procedure would be paid at 100% by the provider (after checking with Angle Health). I need answers since this has been unresolved for over 3 months.
