      Sent me a fake bill via mail, claiming I owed the provider $25 for a visit in 2022. Had me concerned my identity had been stolen, as I have never even been to ************.

      Sent my wife and I fake bills. Everything on them are exactly the same - the service provider (*********), balance, date of service and the type of service are exactly the same. My wife and I would never ever go to the same doctor **** received the same identical treatments. Total scam. DO NOT PAY.

      i Took a COVID PCR test in *******, **, on 2/17/23. I was not charged at the time nor was I told that I would be charged, though I provided my ******** number. I received a Statement nine months later for $149 that said, "THIS ACCOUNT IS SERIOUSLY PAST DUE." I have attempted to reach out to them. I am only given the option to leave a voice message and never receive a return call. This is a very shady business, and I wonder if it is a total scam.

      Sending out fake bills. Not answering phones asking to call back. Then they send you text saying to to pay bill dated this date when you never talked to anyone or told the why your calling. Fake bill

      Took a Covid test 6/30/22.Asked before the test if there will be any charge, the tester said it will be covered completely by my insurance.Got a bill for $130.

      Total scam letter sent out asking for money and a QR code using PREMIUM HEALTH letterhead, ********************************************************************. Phone number ************. SCAM!!!
