      Date services occurred: 4/6/2022 On 4/6/2022, Action Care Ambulance picked up my wife, *********************, from Neuro Restorative and delivered her to **************** Hospital. The ambulance transportation through Action Care Ambulance was pre approved by our insurance. At no time were we given paperwork to fill out or sign. On 6/14/2022 we received a collections notice stating that we had not paid our statement. Upon contacting Action Care Ambulance, we were told that that they, were unable to locate us.We have made several attempts to rectify this directly with Action Care Ambulance, with all communication resulting in being told that we must have some other party solve the problem. All we are asking is for Action Care Ambulance to remove this **** from collections and send it to our insurance company so that we may be properly billed. Action Care Ambulance did not complete their job as required, by not obtaining patient information or a signature confirming any portion of the transport.
