    On 4/25/24 I completed an online medical form in order to schedule an appointment with Leap. Part of the form contained medical insurance information which I completed. I was approved for an appointment and scheduled for a 6/3/24 appointment. I was also approved by Humana my ******************** insurance provider. Before the appt. I was handed papers to sign by **** the receptionist , that said I would cover all costs that my insurance provider would not. I was not given a copy of what I signed. ****************** gave me an exam, x-rays which she suggested I do. I told her that was fine as long as my insurance company would cover it and she said she was sure they would. I told her I needed a new pair of orthodics and what type I needed and showed her my old ones. She said it usually takes 2 or 3 weeks to come in. I paid half of the total $295 and told **** my insurance would cover the rest of it. On July 1st I called to find out what was going on with my order, **** said she would check and said they were in. I asked what my insurance had covered and she told me " you signed a paper to pay in full " , I told her my insurance was responsible for the rest of the cost, she said I was responsible for it and that I needed to bill my insurance after I paid for the orthodics. At no point in time was I told by anyone at Leap ( ***************** or **** ) that I would be responsible for full payment. I called Humana twice and they reached out to Leap and I called Leap on 7/12/24 and left a message but no-one from **** has returned calls. Leap is "in-network for Humana but they have not filed a claim for anything. **** was deceptive in not informing me that they would not accept my *************** plan which should have happened before I even made an appointment. Since I had to travel 2 hours from ********** to ******** and am ********************************************************************************************* good faith. A claim needs to be filed with my ******** health Plan.
