    I had 2 crowns put in on 7/25. I paid ********. Then I got the insurance statement at the beginning of August and it stated that my patient responsibility was ********. I went in to the dentist office on 8/26 and talked with the office manager. I showed her that we were overcharged by ******. She stated that I would get the refund of that amount the following week, but never got it. I called on 9/18 with no response back, called again on 10/14 with no response back. I called on 10/15 and spoke with office manager. She said she should hear back about the refund by that Friday. When I didnt hear back I went into the office again. I talked to the assistant office manager. She said she would have to find out about the refund. Also while this was going on I got an additional bill of 188. The dentist also sent that bill to a debt collector. So not only did I have to take care of this at the dentist office, but also with the debt collector. That was supposedly all taken care of, and office manager said that fee would be taken off my account. Yesterday I got that bill ****** and then another random bill today. I had emailed the dentist office on 10/22, 10/27, 10/28, and 11/5 with no response. Finally today I got an email back that said she took those additional fees off my account that we didnt owe. But we still have not been credited back the $****** that we are owed. On 10/21 I received an email from office manager that said they did not need to credit us back because there was a portion that insurance didnt cover. I called our insurance company and they said that they did cover the crowns and core buildups. I relayed this info to the dentist office that they still need to credit us back the ******, and there was never a response back to that. So on 8/26 I was told I would get that refund, and now its over 2 months later and havent received it.

    We do not have a HIPAA authorization authorizing anyone to speak to you regarding
    ****** *****. Pursuant to the **************** Portability and Accountability Act
    of 1996, no specifics regarding Ms. ***** may be provided to you at this time.
    However, if you will have Ms. ***** sign and return an appropriately executed
    HIPAA authorization, Lone Tree Dentists and Orthodontics and its affiliated
    practitioners would be happy to engage with you regarding this matter.

    I was charged $400 for a one-day crown without giving me the explanation that I could have waited one week for a regular crown which is more durable. I called my insurance and they explained what the extra charge was for. ******, the ****** receptionist acknowledged that that was the extra fee, and refused to reimburse my account. He also refused to provide me with his manager's information and refused to transferred me to the Doctor's ******
