    A surgical procedure was done in 10/2019, over the past 4 years this appliance has migrated and failed to work causing excessive pain and bleeding. Recent appointment was scheduled, co-pay was paid, and an additional surgical procedure scheduled. At no time during this process was I advised there was a money owed or a problem, despite many encounters with the rude, unprofessional office staff. One business day before the new surgery was scheduled, I received a call stating I had a balance that was sent to collections in 2020 and it had to be paid or the surgery would be canceled. In reviewing my insurance claims, nothing was ever billed and all office visits were paid. Billing office representative stated, "well its too late now to do anything, pay the money owed or cancel the surgery". I did cancel and have sought out another provider; however, I have to wait over 1 month for a new patient appointment and an undetermined amount of time before a surgery can be scheduled. Due to the lack of knowledge and professionalism of the office staff, I continue to suffered based on their mistake. This is poor customer service and negligence.

    In November of 2022 after seeing my general practitioner he recommended that I see a urologist for a medical problem that I was having. He immediately send a referral for me to see a urologist at Urology Partners in *********, **. In December of 2022 I had an appointment with ************************ with Urology Partners. ************************ prescribed a medication. The next day I was told by the pharmacist that the medication was not covered and is very expensive and was told to contact my health insurance company for additional help. I contacted my health insurance company and was told that this medication was not covered. They also told me that they have two substitutions for that medication that would be covered. While on the phone with me they called ************************** office and left a message explaining in detail that the medication prescribed wasnt covered but they have two substitutions that would be covered and that ************************ would have to send a new prescription to the pharmacy. I also called ************************** office that same day and have called about twenty times since then and the only answer I ever get is my information will be forwarded to his nurse. Since December of last year now 5 months ************************ and his staff has not yet resolved this issue.
