    I had one visit with this dentist at the office Howell Branch Dental Care. They prescribed me a treatment plan for over $2,000 which involved invisalign, realignment of the jaw, and some veneers/crowns. This visit was on 12/28/23. I completed a consultation and an x-ray. I do not believe I did a mold of my teeth that day for invisalign as I messaged my significant other about an x-ray and evaluation only. They submitted a claim for my x-ray to insurance which states my responsibility as $0. They submitted another claim that same day, 12/28/23, under the code D8090, which is for orthodontic treatment of the transitional dentition, which stated my responsibility as $1,000. I NEVER received this treatment. I was NEVER contacted by the dentist regarding my invisalign arrival and being fitted for it. I opened the CareCredit card to pay for the treatment and have paid $441 so far and am still continuing to be charged. I should not be charged for a treatment I did not receive. I have called the office various times with no answer and left many voicemails. They have not called me back. I contacted the office billing department Thursday 11/14 and she stated she would email me an itemized bill. I have yet to receive that bill. Since then, I have called and left voicemails, but they have not gotten back to me. The date for an orthodontic case is the date the case starts, which is the delivery date of the aligners, which is defined as the day the aligners are fitted in the patients mouth, which DID NOT HAPPEN. Also, other than the fact that I do not owe them $1000, the credit card still has a remaining balance of $2,029. Which service would that be for? There is no insurance claim from this company for a service that would add to that amount. I need this disputed immediately. This dentist is outright avoiding me and refusing to acknowledge my issue. I have contacted them multiple times and they do not reach back out. This is so unprofessional and they have dragged this out.

    This patient agreed to Orthodontic treatment on 12/28/23. (please see attach #2) it typically takes 2-3 weeks for trays to be fabricated and made to give to the patient. We called the patient on 1/9 to discuss a minor tweak the doctor wanted to change in order to benefit the patient. This attempt in contact was made on 1/9, 1/15 &1/4 as show in attachment #2. Pt never called back after three attempts. Pt had and appointment that was cancelled on 2/12 due to her being sick. Pt rescheduled and No call no showed the same appointment on 3/6, 3/11 & 3/18.

    Since then, there has been a new doctor and new management in the office. If pt is wanting to go through with tx we are happy to help.

    If not, we will take proper steps to send undeliverable letter and refund patient as well as insurance.
