    On June 28, 2023, my 85-year-old dad was referred to the Fernandina Beach Rehabilitation and Nursing Facility (“facility”). When we arrived at the facility, my dad and I were taken to his semi-private room. While waiting for over 30 minutes for someone to talk with us, I inspected the bathroom, which reeked of urine and had ***** matter around the rim of the toilet. When I inquired if a private room was available, I was advised to speak with the Administrator. The Administrator moved my dad to their best private room; however, the leather reclining chair was worn and torn and too low for my dad to sit in (I refused to sit in it without a sheet cover), the serving tray was chipped, and the toilet seat was discolored from white to brown. The next day, my dad told me that on three separate occasions during the night, he rang the bell for assistance to use the bathroom. On all three occasions no one came. Because he did not want to wet himself in the bed and sit in his own urine, he got up to go to the bathroom on his own. This despite wearing a bright yellow fall risk band. At that point I felt it was unsafe for my dad to remain at this facility. But more alarms became apparent. At 9am, I informed the Administrator that I wanted to take my dad home, and would need in-home physical therapy and I provided her with the phone number and fax number where the referral would need to go. At around 3:30pm, I had the discharge papers. However, I was shocked when on June 30, the in-home rehabilitation office informed me that they had not received a referral from the facility. I could not believe that this facility released my 85-year-old dad without a referral for neither in-home nor outpatient physical therapy. Luckily, on July 1 my dad’s primary care provider provided the referral for outpatient therapy; and on July 5 the surgeon provided a referral for in-home therapy.
