      A staff member contacted the ****************************************** to report that I was giving my child a controlled substance. I spoke to a nurse who collected the information of what medications my daughter is taking, including the dosage, number of times she takes them daily, and the pharmacy where the prescriptions are filled. I was contacted later the same day and asked the same questions because they could not locate the information that was obtained earlier that day. The following day I received an unexpected visit from ************** of Children and Families due to a report from Saluscare staff that I was medicating my child with a controlled substance. My daughter is prescribed the medication. I also filled out a petition to have my child released from their care on the evening of 2/4. When I called the next day to inquire about my childs release, the staff informed me that they misplaced the form and they werent sure why it wasnt seen earlier. This facility is not well managed.

      Blessings. I have used every means offered by Saluscare to obtain my records and it has been a week with no response or records received. Suffering from severe PTSD is not helping as this matter causes high anxiety and they are not helping nor do I have the ability to walk in and obtain my info.
