      This business has a poor system for medical records request for your children. They have a website with a tab for providing a release to obtain the medical records, but the form doesn't have a place to insert the information for the parent for purposes of obtaining the records (other than a street address). Also, the staff are rude and rather than assist you with obtaining your medical records (or understanding whatever process they use), the staff immediately try to transfer you to another part of the company until the call drops. For example, I spoke to their Medical Records line, who then transferred me to the South Tampa Office, who then transferred me back to the Medical Records line where the call then went to voicemail with a full mailbox. It is very disappointing to have to deal with this incompetence when it pertains to something such as your child's medical records.
