    Date of service March 06th 2023 , cost $6000.00 cash) Seen by Dr. ***** on 3/6/23. Im not satisfied with the dental work I received at clear choice in **********. I went to clear choice due a broken front tooth. On the day of my first visit I was explained the procedure and cost. (remove of tooth, receive an implant , abutment, crown) on March 6, 2023 a lot was and I was feeling the excitement i cried knowing that i would be able to smile again. Next visit June 10th told gums are not ready. Return on August 10th 2023 ( wrong tooth size and color) told my order/tooth would be expedited, which was not so. Next appointment was August 28th which was cancelled. Not sure the actual date I received my implant. But I remember not being excited at all. I just wanted to cry! I was not happy with the tooth (the size or color and it just looked to big)i expressed my concerns ,when back to the office a few times explaining the discomfort. Last visit was 02/2024 with Dr ********* I explained the tooth was to long and it was causing discomfort due to the length of the tooth. Doctor ********* stated to file down the tooth Just a little stated she didnt want to damage the tooth. I explained that it still looked the same and she blow me off and said there was nothing else they could do that. I needed to go see a dentist. So on June 27th 2024 I took Dr ********* advice to dentist due to the discomfort and being so unhappiness with my smile. This has caused me so much stress and ***************. Im now always covering my mouth not to show my teeth. Im requesting a partial reimbursement of the $6000.00. Which is $3000.00 dollars to cover ( ********************* of my current abutment and new crown) I deserve to have a beautiful smile! I can also provide pictures and X-ray if needed.
