    Late August, 2022, I had an eye exam by Dr. Mark **************. Today I went to retrieve my prescription and the staff at *** ***** ****** *** *** said the Doctor left and took all of his prescriptions. I called a number which connected me to a local Walmart and the doctors assistant said the prescriptions have been lost or damaged.

    It is correct that I did leave *** ***** ****** *** *** September 2022.  As is required by law I did take my patient charts with me.  However, it has ALWAYS been my practice to provide each patient with a SuperBill that has the ICD codes, amount billed and paid and the prescription for the patient's records. *** ***** ****** *** *** also made it a requirement to scan this information to their patient file.  He should have easily been able to get a copy from them if he had misplaced his original provided at time of check out.  I have asked repeatedly that any and all request for records or Rx be sent to my email address [email protected] with the patient's full name, date of birth and the approximate time of visit as this is more efficient then calling and the confusion of what the patient needs and from which location said patient was seen.  The 14 filing cabinets housing the patient files from *** ***** ****** *** *** are in a secured storage facility until I can have them separated and dispersed to my current 4 locations.  Mr. ******** was informed of this and demanded his information immediately.  All previous request for information from other patients had been met within 3-5 business days at no charge to the patient.  This was offered to Mr. ********, and he declined and threatened to report to BBB.  

    In NO instance did anyone say or should have said that his information was missing.  It was just not attainable at that given instant.  Perhaps there was some confusion or miscommunication between Mr. ******** and the Walmart optical person he was speaking with.
