      My son was seen at the location on March 18th to have a form completed for his school for special education services. Because the provider we saw was a physician's assistant and the school required signature on the form by a medical doctor, the form was left with the office to have the doctor sign the following week. I was told they would call me when the form was signed. I called the office on March 24th because I had not heard from them. I was told that someone from the forms department would call me back. They did not. I called again on March 28th and again was told a message was being left with the forms department. A women named Lisa called me back and left a message but did not give a status of the form. I continued calling her and leaving messages for several days. On March 31st I spoke with her finally and she said that form was signed but that there was a $25 "forms fee". I was never told about this fee previously but paid it because this form is required by my son's school. (I had already paid $103 for the appointment which the sole purpose was the signing of this form). She said she would process the payment and fax the form to me by the end of that day. When I had not received the form on Friday April 1st, I called and left another voicemail asking for the form. As of today, April 4th, I still have not received it.

Quality DIR Contact Phone: ************ Contact Email: ********@gcmc1.com Good Afternoon, I have reviewed patient's mothers complaint and these are my findings: Mathew was seen on 3/18/2022 by a nurse practitioner. The form needed to be signed by a physician. The form was scanned into the patient's chart and never sent for signature. On 3/24/2022 patient's mother called requesting the form be emailed to her. I cannot find where this message ever went to any employee but stayed in the patient's chart, patient's mother called requesting update and the message went to Lisa. Lisa called patient's mother at 5:52 pm and left VM to return call. She had Dr. sign the form on the 29th and faxed the next day. Patient's mother called on 4/12/2022 due to the school not receiving the form. On 4/13/2022 Lisa refaxed the form to the school again to a different fax number after contacting the school and finding out the fax number on the form was incorrect. Lisa then contacted the school and spoke with Julia P. to make sure the form was received and left a VM for patient's mother. Lisa also left another VM on 4/14/2022. I agree there were several issues surrounding this form which have been addressed with the staff and offer my apologies for. I do not feel there should have been a charge for completion of the form and I am putting in a request for a refund. I would just like to clarify a few things from your findings. I asked the front desk staff when I we checked out if the form would be faxed to the school. They told me I would need to pick it up. I was anticipating a call for that which is why I kept calling and following up. When I spoke to Lisa on the phone when she took the payment, I asked if she could just fax it to me instead of me picking it up since so much time had already gone by. She said she could fax it to the number on the form and I asked her to fax to me instead so that I could be sure it got to the school as I am in contact with them often. For some reason she still faxed it to the school. As far as Lisa leaving me voicemails, she just asked for a call back, never providing any information. And when I would call, she never answered to responded to my voicemails. I left several. Again, I appreciate the understanding and the refund. We greatly enjoyed meeting with the provider, Bernie, but after this experience will be looking for another physician for my son.
