    My step-son was diagnosed with testicular cancer in April of 2022. In conjunction with his diagnosis and treatment he had two CT scans performed by St Johns Radiology Associates. 4/18/2022 - $325 4/19/2022 - $177 At the time of the scans, my wife was present with my step-son. They provided our insurance card and our step-son's drivers license before the provider would perform services. They performed the work properly, but did not bill our insurance company properly. We called several times and provided them the proper information. We also called with the insurance company representative to fix the problems. In addition, I updated the name and insurance information online on their computer system, but they continued to overwrite or not accept the correct information into their system. The billed the services using an erroneous name for my step-son, the wrong insurance company, and also indicated that my step-son was the policy holder and not my dependent. They sent the 4/18/2022, $325 for collection in December. They never contacted about us the inability to bill the insurance properly, other than sending a monthly bill that did not reflect any changes that we discussed with them over the phone.
