    Turner D, is the owner of the company and is located in Tallahassee, Florida, Pharmacist, NPI#*********. I have left him several messages and he has not responded. I began the First of Six Infusions on March 21, 2022. I paid $300.00 per Infusion. Which were $1800.00 in Total for the Six Infusions and six Therapy Sessions. The Infusions and Sessions were to help with my Diagnosis of Severe Depression and Anxiety. I was told during the phone-consultation that I would receive an Intravenous Infusion for $125 and an Outpatient Visit for the evaluation of my symptoms which would $175.00. totaling $300.00. (as stated on my Superbill (Attached). There were two women in the Office. Heather B, rendering provider NPI ********** ( The woman who gave me the infusions.) The other woman in the office worked for another company that the owner of New Horizons Infusions Clinic owns. Some sort of a Marijuana Clinic named Diana. Diana solely received my payments and had me sign docs that I was not pregnant. I never once received an Outpatient Therapy but was given a name of someone I could speak with privately at my own cost. During one of my infusions Ms. B. may have inserted the needle wrong and when she came back into the office 15 minutes later my arm had severely blown-up. She put a heating pad on that arm and inserted the needle into my other arm. I must of been receiving too much Ketamine because I became very sick. Unfortunately, the next patient arrived and they would not let me stay in the chair. I was asked to go into Heathers office and sit at her desk. I was too sick to sit so she had me lay on the floor for an hour. After my last Injection there was no follow-up phone-calls. I called the Clinic and was told Heather no longer work there. I found Heather by calling 14 people with her name. She advised me many people complained and that's why she left. She was basically the only one working there. I have Text-Messages to prove this.
