    I have requested twice in writing to receive an itemized bill for a Doctor ordered PET/CT Calcium Scoring. Letters dated February 14 and March 5, 2023 for services rendered on November 21, 2022, wherein both letters from me requested that I need an Itemized bill (description of service rendered, my Patient name, date of service and Provider information) included a self-addressed, pre-stamped envelope for Imaging for Life (also known as MAXhealth Radiology). I do have a "Receipt" that I paid $60.00, however, my ******* Cost Sharing plan (*******************************) requested that to process my entire Annual Physical costs (which total over $1,100.00) can't be processed until I provide them the $60.00 Itemized Bill from Imaging for Life (which operates as MAXhealth Radiology). So, I have a receipt, but that lacks what the receipt is for. I've kindly sent two letters, including the Notice from my ******* Cost Sharing Plan requesting they need the Itemized Statement for services (I have a receipt) rendered so that I can be reimbursed for my Annual Physical which I pay out of pocket but get reimbursed for after submitting required medical itemized statements and receipts. I just need to receive it an Itemized Bill for ******* Services Rendered. It's been over a month requesting same. I'm visiting my son up North so if it can be mailed to: ****************************************************************************. The address I just provided to you was on the two Pre-Addressed, Pre-stamped Envelopes already mailed to MAXhealth Radiology. I just need them to send the Itemized Bill in the US Mail.     We never received letter to our facility.  Attached is the ledger that patient is requesting.  We will be mailing it to patient's requested address on Monday as we are closed today.
