    My father passed away Thanksgiving Day, 11/24/2022, we visited the office 11/25/2022 to arrange for cremation services which cost $1,000+tax. We spotted an error on the state form, his mom's name was written wrong, instead of **** they wrote ***, it was ****** out & rewritten. On the 29th we discovered they placed his dad's middle name on the form instead of his, the funeral director ************************* became defensive & combative, stating it was our fault for signing the forms. I told him it was a clerical *********** sure it could be corrected. We met with him December 5th to pick up the certificate of cremation so to proceed with the burial services, we text him copies of my dad's driver's license and birth certificate to show his correct name to accompany the amendment being submitted to the ****************** we later text him his green card as well.My dad's service was held on the 28th, on the 30th of December a follow up call was made. The secretary said nothing was done because we never gave them the needed information (the ones text directly to the director). The same documents were emailed to ****************************************** and given 2wk timeframe for the corrections.My mom was called on January 16th & told the corrections were ready for pickup but with a $38 charge to the funeral home that needed to be reimbursed. Upon arrival, **************************** lied and said he misspoke to my mom; that the death certificate needed to be picked up at their health department office. The secretary corrected him and said it, the corrections, were never sent in. She stated the additional documents weren't sent in until the 12th of January, I corrected her & showed her my phone to show the ************ log.I was given a copy of the original submittal and noticed additional errors.On February 2nd a complaint was filed with the **************** of ******** ******************* Services. We simply ask to have their mistakes fixed so we can take care of my father's estate.
