Have been trying for 3 months to get this practice to send a Pryor Authorization request for my daughter. Insurance company keeps telling me they never received the form.Business response

Business Response /* (1000, 5, 2022/10/07) */ We have sent the response back to ********** 4 times. ********** has never said they wanted ***** until fax number 3 was received back from them. My Triage employee has spoken to Mr. ***** everytime to let him know it was sent. My Tirage employee speaks to ********** and they tell her they have the faxes. The last fax, #4,sent was the ***** for the medical equipment, that have a code. Mr. ***** was infomed of this as well. We have emailed him today everything that has been done. We have exhausted every avenue to try and help get this resolved for him. *********** does NOT tell you what they need, they just say addition information is needed. At no time was a ** ever asked for from *********** On the 3rd fax from them they finally said *******
