    I received a bill and on the bill it states this is a temporary statement until further notice and no information on what I am being charged for but I owe $50.00. It is coming from **************, ** and this is odd because I can&#**;t even call them to get any information about this bill. They having me mail the bill in ********, the company is listed in Cedar Rapids, ** and the payment is to be sent to *********, **. I would like to be contacted by the business about this.

    We connected with this patient on May13th. The patient stated that she was just a little confused as to why the bank address and return mail address were not in the same location as the hospital. This is because payments are sent to a bank address and mail returns are handled by another vendor. The patient requested some further details about her services which we were able to provide. At the end of the call **************** was pleased with the response and information provided.

    Our customer service team did pull reports to determine if **************** had tried to contact us prior to logging a complaint with BBB. Based on the patients phone number we were able to pull the following information:

    04/23 @5:30pm Patient attempted to call after hours but did not leave a voicemail
    04/25 @10:14am Patient called and was in the queue for 2 minutes before hanging up
    04/25 @10:17am Patient called and was connected with representative but she did not want to provide her name or DOB so the representative could access her account
