    GoForward abruptly shut down its mobile app and canceled all patient appointments on or around November 12, 2024, stating that their medical team would be available to support patient care via email until December 13, 2024. The app is the only location where patient could access his monthly billing statements and certain medical records in chat logs relating to patients medical care by his doctor. GoForward has been unresponsive to patients emails requesting invoices/billing statements for the period July ***************************************************** the ********************** app chat logs. Patient has emailed GoForward about this request approximately 20 times between November 13, 2024 and December 4, 2024. This matter is time sensitive because the company is discontinuing operations and has announced it will stop supporting patients on December 13, 2024.

    Around April 15th I ended my subscription with GoForward as my primary care provider. I had the subscription for over 8 months.The company cancelled my prescription refills so I am no longer able to get my medication. I need to see another doctor to maintain my heart and other medication but have 6 days to book an appointment and get my medication before I am lapse and at risk.The other services rendered were unsatisfactory which is why I cancelled initially. Private Equity should stay out of health.

    I have been a member since May 24, 2022, 12:26?PM. The company claims I can have an in-person appointment whenever I want, although, there are NO appointments available at all. They do not answer their phone, it is a recording telling you they don't. They only want to communicate via app or email. The chat app person I am dealing with refuses to tell me why there are 0 appointments available. The chat people want to send me to urgent care for another fee which they should be seeing me for that I pay $149/mo. They refuse to answer questions about why I can not go in person. This is completely opposite of their tv ads, their claims on their website and anything at all. A full complete refund is owed. They say you can cancel after 6 months but no way to easily cancel because they do not respond.

    I have emailed and called in several times to no response. I never wanted this services. It has been weeks and i have been charged multiple times. I have requested cancelation multiple times. I am on fixed income with high ins coverage. I do not want this service nor did i agree to it or use it in any manner yet i am being charged. I want my money return and service canceled immediately.
