      I have been working to resolve an issue with my preceptor's license. It is difficult because she has renewed her license but the ************************* (****) has been hacked and short staffed. As a result, her license has not updated on the **** website. My school has not been understanding of this dilemma and will not approve my preceptor for my final class needed to graduate. Finding a new preceptor with less than a week to spare is not possible. My school has claimed they will will accept a receipt that shows my preceptor had her license renewed, but then they went back on their word and refused to approve my preceptor. I will attach a photo of the **** hacking evidence. I will also attach a photo of the school stating they will accept receipt of license due to the **** being hacked.

      On behalf of Chamberlain University (Chamberlain), I am responding to your request dated February 23, 2022, regarding Chamberlain student, ***********************************.

      Family Nurse Practitioner (FNP) preceptors for the Chamberlain FNP degree program must possess the following, as outlined in the *** FNP Practicum Handbook:

      a master's degree in nursing at the minimum and hold an active, unencumbered advanced practice license which corresponds to their area of clinical practice in the state where you complete your practicum hours. The preceptor must also have at least one year of practice as a nurse practitioner. Other types of health professionals may also serve as a preceptor for FNP students: (e.g., Medical Doctor [MDI or Doctor of Osteopathy [DOI). MDs and DOs must have an active medical license.

      Additionally,two sessions prior to the start of a FNP students practicum course, the application for the student proposed curriculum site, preceptor Curriculum Vitae and preceptor license must be submitted to **********************.

      On December 2, 2021, Chamberlain attempted to contact Mr. *********** preceptor,Mrs. Akukeng *** to notify her that her APRN license was showing as expired as of November 28, 2021, with the *************************.  Multiple additional attempts were made to Mrs. *** for updated information on her license.

      On January 12, 2022, ************************ was advised that his preceptors license had expired, outreach to Mrs. *** was unsuccessful and was asked if he would be able to obtain a copy of her updated license. From this date, January 12, ********** present, Chamberlain and ************************ have been in contact with each other to try and obtain a copy of Mrs. ***s updated license.

      ********************** was also made aware that the ************************* was involved in a security incident and as a result they were not able to process renewal applications.  Per the ************************* announcement,

      Finally, please be advised that your license or certificate will NOT remain active if you fail to submit a renewal application to the Board at least two calendar weeks before the end of the 30-day grace ****** following the expiration date of your license or
      certificate.

      On February 10, 2022, ************************ asked if the preceptor could send a picture of her mail to show her renewal.  A Chamberlain colleague stated that if an official copy was received, it could be reviewed. On February 17, 2022, ************************ provided a picture of the preceptors written receipt from the *************************.  On February 21, 2022, Chamberlain notified ************************ that the document would be sent for review.

      On February 22, 2022, Chamberlain notified ************************ that the documentation he submitted could not be accepted and he was asked if there was any other documentation that he would be able to submit. Mr. *********** preceptors license had expired November 2021 and she did not submit for renewal until February 2022, which did not follow the ************ renewal process. The renewal date on the receipt is outside of the two weeks prior to the 30-day grace ****** from the license expiration date, as outlined as the license renewal deadline by the *************************.

      *********************** wanted to appeal this decision and the Associate ****, Post Licensure Clinical Operations was notified. On February 23, 2022, the following conditional registration options were provided to ************************ to begin the March 2022 session with his current preceptor:

      Student may not start with preceptor until license is active
      Student should know that if they start the course and do not finish they will still likely be financially responsible, and if they are not able to complete the clinical hours they will fail the course. All course policies will apply. They should also consult with student services for all financial and academic ramifications before start gin the course.
      All hours must be completed within the timeframe of the course and an incomplete will not be granted if the minimum practice hours are not met.

      Chamberlain cannot approve Mrs. *** as a preceptor, until she has a renewed, current,active, and unencumbered license on file.

I had some financial issues and was delayed in paying off my loan to them but have paid them and ***** and they will not supply me with my masters degree certificate. I have asked repeatedly for the last five months they keep on promising and it never comes. I paid over $20,000 fir this degree.

      On behalf of Chamberlain University (Chamberlain), I am responding to your request dated January 16, 2022, regarding Chamberlain student, *******************************.

      Upon conferral of Ms. ********** degree she had a balance on her student account,of which she owed.  Per Ms. ********** signed Enrollment Agreement, Failure to fulfill all financial obligations may also result in denial of student's access to grades, diplomas or transcripts.

      A payment of $1,159.33 was made on September 22, 2021 creating a zero balance on Ms. ********** student account balance and removal of the hold on her account.  An order for Ms. ********** diploma was placed on October 11, 2021, with Chamberlains third-party diploma processor,Parchment.  After reviewing the order, it was determined that an error occurred with Parchment and the hard-copy diploma was not sent to **********************.

      Last week, a second diploma order was placed for **********************.  The e-diploma has been claimed.  Per Parchment, the hard-copy diploma should be received by ********************** by next week. Should it not be received within the next seven-ten business days, we ask ********************** to contact Chamberlain to request a new copy.

      Chamberlain has implemented a Covid 19 vaccination policy. I fulfilled all the requirements needed to start, including vaccinations, physicals, drug testing, and CPR. I find it unfair that those requirements are now changing after what I already agreed and signed to. This is a breach of the contract I signed. I am being denied a full refund with zero explanation as to why and am being passed from person to person, with everyone telling me it isnt within their scope to ***** that. I am escalating this issue as far as I can. I am forced to choose between my body and the money invested. It seems this policy has been in the works for some time. I should have been made aware that Chamberlain would implement this policy before signing up for the program.

      I paid an outstanding debt to Chamberlain University to re-enroll in their RN-BSN program. I did everything that was required. Now, I am being told in order for me to re-enroll in the school that I would have to pay the full balance. I have a screenshot that states to take the collection up with the collection on Chamberlain's student portal. It does not state in the portal that whatever I work out with the agency will only result in me getting my transcripts. I also have a screenshot from the collection pay stub that states, "full and final settlement payment equal to 75% of the current total balance due as listed above. Not one person from Chamberlain stated that if I settle for 75% of the balance I will only get transcripts. All I was told by Chamberlain reps is to talk with the collection agency work it out with them. I did that now I am being told I have to make an additional payment. I talked to the agency the account is now close, because payment has been made with them.

      *********** was enrolled in the Bachelor of Science in Nursing degree programbeginning in the January 2017 session. She withdrew from the May 2017 session on May 31, 2017.  At the time of her withdrawal fromChamberlain, ************ had a $2,815.45 student account balance. 

      Page189 of the Academic Catalog states, Delinquent payments may result inregistration holds and loss of ability to participate in payment plans. Anystudent account balance owed when a student leaves Chamberlain must be repaidto Chamberlain within four months of the date attendance ceased, in accordancewith terms of Chamberlains student account agreement.

      Andper ********* signed Enrollment Agreement, Failure to fulfill allfinancial obligations may result in a students account being reported to thecredit bureaus and denial of students
      subsequentregistration, access to grades, diplomas or transcripts.

      BetweenJune 29, 2017 and November 23, 2017 Chamberlains ********************** made tenattempts to contact ************ regarding her balance, either by phone or bymail.  On November 23, 2017, *********student account was placed with an external collection agency.  Her account was placed with a second externalcollection agency on February 1, 2019 due to non-payment.  

      Mostrecently in 2021, ************ settled her account with the external collectionagency.  With that settlement, *********** able to obtain her transcripts. However, the settlement did not entail thatMs. King would be able to re-enroll at Chamberlain without paying her remainingstudent account balance of $653.86 to Chamberlain.  ************ has been advised multiple times byChamberlain representatives that she would need to pay the remaining balance onher student account in order to apply and re-enroll at Chamberlain.

      I have tried to obtain financial assistance. I am in need of a scholarship that will help with current and future education fees from the school. They only have incentives for incoming students which I think is discriminatory for students who are currently struggling with finances within the program. They never offer scholarship opportunities for current students as a continuing student education relief. I do not have any funds to continue the program and have already sat out a session. If I do not have a scholarship soon, I might have to sit out INDEFINITELY. I have tried to contact the finance ***** they only offer external scholarships which are hard to obtain because of the large pool of applicants. I can't get a federal or private student loan because I don't qualify. Also, they have not tried to cancel student debt for the 2020-2021 year while COVID occurred like other schools. This is an institution issue that needs to be addressed overall. PLEASE, I NEED HELP!! ASAP! Thank you.

      Ms.Oshun began her enrollment in the Bachelor of Science in Nursing degree programin the May 2020 session.  She mostrecently completed the May 2021 session, which ended on June 26, 2021. 

      Priorto her enrollment at Chamberlain, ********* attended another university andcompleted a bachelors degree at that university.  While at that university, she utilizedFederal Title IV funds to pay towards her enrollment.  On April 23, 2020, ********* met with aStudent Support Advisor (***) at Chamberlain to review her EstimatedFinancial Plan (EFP).  She was advisedthat because she had used Title IV funds with her prior degree, she only had$8,194 in Title IV funds remaining.  Duringthe meeting, the *** discussed the out-of-pocket costs associated withattending Chamberlain and provided information about an internal grant.  She was eligible to receive a GAP BalanceGrant of $500 per session and had been receiving that until the end of herenrollment.

      Duringthe last year, she utilized the remainder of her Title IV funds.  Specifically, on March 10, 2021 she wasnotified that she had met her Federal Direct Loan aggregate loan limit. She hasbeen provided information on external scholarships and private loans.  ********* has stated that she is not eligiblefor a private loan.  Chamberlain offers aCredit Extension program; however, the main requirement is that a student hascompleted at least 75% of their degree program. Unfortunately, ********* hasonly completed 66% of her degree program.

      Chamberlainhas continued to provide ********* with external scholarship or private loan websitesand will continue to do so.  She isencouraged to review those websites and reach out to her *** with anyquestions.
