      I want to file a complaint against ***** and Midland Orthopedics and Associates for charging $500 to my HSA account without my authorization. I clearly told ***** who called me on 2/13/24 regarding a bill I wasn't aware of and had not received that once I get the bill and reach out to my ******************* that I would pay it myself. She advises that once I get the bill that she stated was mailed out on 02/13/24 I could call to make payment arrangements as well. She had mentioned the bill was $1005 and I had questions. ***** then proceeds to call me the very next day 2/14/24 around 12:30pm asking if I spoke with my health ************ and I told her no and that I'm still waiting to get the actual bill. She says okay and hangs up and not even 20 minutes later I get a notification on my phone that $500 has been charged to my HSA Card. I go log into my account and see the charge is pending. Then I get another notification that a charge had been declined. They used an old card on file to attempt to take a payment because they never had my new card on file. When she called me on 2/13/24 about the bill I told her I would mail, I assumed she was referring a bill that was for $139 she called about before and that's when she stated it's for another bill I hadn't even received yet. They went and found the $139 bill I paid back in January to get my new card information and charge that $500 without my authorization and I'm very upset about it. I spoke with both my HSA and BCBS reps and they also stated that they should not be charging my card without permission/authorizations. And when I called ***** to ask did you just charge my card after I told you I would pay it when I get the bill and why did she do that, she proceeds to tell me she was told to do it. This is the 3rd time she keeps calling me to get payments over the phone for bills that they never mail to me and give me a chance to pay. I don't think they should be taking upon themselves to charge my HSA card.

      I had an appointment with Midland Orthopedic Associates on Feb. 2. ************************************ told me I need to have a MRI done. He asked me which location I would like to have it done at. I made it clear I would like to go the **************** on 93rd St. I called **************** to make an appointment and I was told there was not and order for so they could not make an appointment. Advocate did mention that they had a referral for me. So Midland faxed a referral over to Advocate but did not fax the order over. I placed a call to Midland to see what was going on and they acted like they did not know what I was talking about. Eventually I was told that they did not know the fax number (Which does not make sense to me, because they faxed over the referral). So I called Advocate to get their fax number (I feel like the receptionist should have done that) and I called Midland back and gave the fax number to the receptionist. Several hours later I called Advocate back to make an appointment and again they said they did not have an order for me. I called Midland again to ask that the order be faxed over and I waited several hours before calling Advocate again to make an appointment. Once again they said they had not received an order for me. So I called back and asked to speak to the office manager. Went through the same process with him and he said he would call me when it was sent over. I received a call from him stating that it was done, but when I called Advocate for an appointment they said there was no order for me. Advocate said that maybe it will come in later that day. I waited until the next day to call and once again they said they did not receive anything for me. So I have waited for a month to have this order (not referral) to Advocate and they have not done it. The last person I talked to at Advocate that they have a referral for me and nothing else. Me and my insurance company paid for this visit and I did not receive the proper service.
