    My Alsip, IL mailing address was changed without my request or consent to *******, **********. My medications were then shipped to *******. I contacted Hines VA pharmacy about needing my medications and they alerted me that my mailing address was a ********** address. I had them change my address back to my Alsip Illinois address.I spoke with Patient Advocate **** and he told me he could contact Privacy Officer to speed the process so I could get a list of everyone who accessed my medical record. **** told me there wasn't anything he could do to ensure this incident wouldn't happen again. I want a lock of some sort put on my medical record and I want disciplinary action taken on the person who changed my address without my consent.

    I have asked them to come remove one of their patients from our home .and they keep telling me he isn't their Responsibility when he is .He is causing alot of problems in our home and is nothing but lazy won't help around here and he has been asked to stay off the couch and he won't His name is ********************************* I have asked them to come remove him by the weekend And they won't help me ***************************

    I am asking them to come remove someone from our home that is living with us that is a client of Hines hospital Hines ** Someone we know works there and she said she would come remove him from our home and now they are saying they won't help me when he is there responsibility .Not ours .he is causing problems in our house and I want him gone .***************************

    They won't help me remove one of there clients from my home. I have asked them to remove him from my home and they said he isn't there responsibility. That they won't help me .That he is causing problems in my home .and i want him gone ***************************
