  Why is this kind of insurance scam allowed in *****************? Our country is better than that. It is frightening, and has to be illegal. All the same deception I have read on every single review page below, is my same story. My wife and I had two policies with them, the Smart Health Plan 300, and another one that was with Unified Life that we thought was a secondary (Which turned out to not be insurance at all, more like a discount plan). Both plans had a combined cost of around $750 per month, and we had been paying it for years. I am now strapped with $93,000 in medical bills, all of which Federal Life paid nearly nothing. Below is a breakdown (Smaller ones not listed because they paid an insulting amount on those):So, for $750 per month for years, on coverage we were told covered 70% of almost everything we would ever have to medically worry about (especially on unexpected hospital admissions and surgery), would you call the breakdown below... Insurance at all??? Do you really think a sane person would sign up for the coverage below, at a rate of $750 per month, with two different policies, and expect the coverage below... Without being lied to?If this company had told me that for $750 per month, they will cover only 10% of all medical procedures you encounter... I would have NEVER taken the policy!!!! Nor, would any sane person...EXAMPLES:Lab Work Cost: $1001 Insurance paid: $100 __________________CT Scan Procedure: $3054 Insurance paid: $305 __________________Hospital Admission Surgery Unit: $12,852 Insurance paid: $1421 __________________Electro - Physiological Heart Surgery: $73,801 Insurance paid: $7380 __________________ER Visit + Admission to KDMC: $15,299 Insurance paid: $1630 __________________

  My medical bill wasn't paid at all. I just received a letter from them (after just 2 weeks ago, they'd withdrawn my monthly premium) saying they're terminating my plan. I called and the company is closing down. They're no longer in the medical coverage ***** This letter was dated December 27th 2024. They withdrew December, January AND February's premium. Was this just a fake company? Have we all been duped?

  March ****** I called this company two weeks ago and could not reach anyone because I received an email from them stated that my policy is now with a new provider. So, Friday February *******, I called and talked with a representative. I begin to ask questions. Do I have a health policy are is this a discount program. The representative told me that she would be glad to go over my policy with me.And that she would send me a copy to my email address. She confirm the email address, but I have yet to receive a supposed copy of the health insurance policy. I called back several time and finally I told them to cancel. This is not a legit health insurance company. It is a discount company. I have never used the discount card. The company just seen suspicious. I will continue to research to find a true health insurance company.

  This company is a complete fraud. was sold this plan thru florida marketplace. Have the ********* 100A plan. you don't get to speak with anyone on the phone and when you ask for call back as they promise you do not get one.. Have paid for almost a year and get no claims paid!!Will cancel as soon as i can. Wish I could give less than 1 star!!

  Paid them over ******* and had 1 claim, doctor tried collecting ****** and after 18 months of trying they did not paid and know ask me to pay. Called and cannot get thru to anyone, no one was in que for call and they still did not answer after 15 min on hold

  SCAM!!! After paying $800 per month for several months the medical bills started rolling in. They paid nothing. We got the run around for months and had to pay all our medical bills out of pocket. We lost $5600 in premiums and had to pay thousands to providers. They agreed to cancel our policy and pay back our premiums but it has been two months and we have not received a reimbursement and they are not answering or returning our calls.

  This is a scam! Do not waste your hard earned money on this. We have been told by multiple Healthcare providers that this is not an insurance policy but a discount card. Had since August and have not been able to use once. If you are able to reach someone and that's a big if they give you the run around and are completely unprofessional. They don't have all the notes and lie, lie, lie! Wish there was capability of leaving zero stars!

  **** limited med is underwritten by federal life which did not cover any of my medical bills from an emergency room visit leaving me with over $12,000 in expenses to pay. When signing up for the insurance, I was promised no out-of-pocket expenses for any medical expenses. When contacting the insurance company they said they would cover the expenses. This was the first phone call several calls and hours waiting on the phone on the line, they said they would Cover 70% of the bill. Now they have flat denied the payment and will not cover any of my emergency room visit. I have not received an itemized bill from them or any correspondence at all when I ask them for this information they will not send it to me. This is a scam and I have been taking advantage of.

  Scam!!! First of all if you can even get a person on the phone , you are told different things with each conversation. I was initially told the coverage for ER, Xrays, Md visits, hospital, surgery, ambulance would be 80/20 Then I was told 70/30. I received my first EOB. Looks as though Federal Life pays the 30% and the 70% is on me. The first EOB is one of 3 thus far. They have only pd on one. Not paying the other two. I had an accident which is costing well over $110K. The billing person at my Physical therapy office spent 2 days on hold for 2hrs each day in an attempt to get information regarding coverage! She too was told different things when she was finally able to speak with someone.

  Like many of the comments below this is the worst health insurance. They took advantage of a 20 year ******** veteran who needed insurance. We were paying almost $600 a month not knowing they were not paying anything. Many doctors tried contacting them about very important and time sensitive medical procedures and they never responded. They never respond to their email or the phone number. I am still paying for medical services that should have been covered under this insurance. These medical services are in the thousands! Very disappointing that this is happening and to our veterans! I also notified my member of ******** and my Senator and informed all of our military friends to never do business with you
