      Looked up free covid testing site and Center For Covid Control came up. Went to the location at ************************************************ for a free test. They required a lot of personal information to get the free test. Requested social security number, drivers license number, insurance along with name, address and email. We completed the test and the test results were sent via email, but the testing times were not accurate. We went at the same time and the test time were hours apart so I don't think they acutally even processed any tests. The more concerning is the amount of personal informatin we provided and what they will do with it.

      I went to the Chinatown location for Covid testing on 12/20. I requested a PCR test and was told I would have to take both tests and couldnt just get the **** I took both and got the rapid test results that night, negative. It is now 1/17 and I still do not have PCR results. Ive tried calling but face 3 hour wait times with hundreds of callers waiting on the line. After hearing of the complaints, I wanted to share my experience. Be cautious. This place no has a copy of my ID and insurance card. Thankful I left the *** field blank when registering for my tests!

      I have been to this testing location 2 times to get tested. I have since found out they are not cdc affiliated. This "company" now has my insurance and address information.

      Never received PCR Covid results, almost 3 weeks!multiple attempts to contact, 800 numbers not taking calls, 3 hour wait times. Last week a staff member answered local number and told me to text my info again for lab. Still no results. State should not allow this facility to operate.

      The place advised to offer free antigen test for COVID. However there are multiple red flags about the place that raises questions about their integrity and practice. The set up is not professional and it's not clear if the facility is accredited, and if the test results are reliable. It's also not clear if people work there have proper training and are healthcare professionals. The test results are promised to be available in ***** mins, but it has been more than 4 hours. The business also required uploading insurance information as well as a photo of ID (i.e., driver's license) but there was no verification of identity when present for testing - highly suspicious that this is an information collection scam. This place is very likely endangering public by offering scam test for COVID. Please investigate and require the business change and to delete whatever information they have collected from the public.

      Hello,

      Our staff is fully trained to provide clinical laboratory tests, specifically for and beyond rapid antigen and PCR COVID tests. They are certified through the US Center for Disease Control and Prevention, and the World Health Organization. 

      As for gathering photo identification and or insurance information, it is Doctors Clinical Laboratories policy to verify patients identity to make sure it matches their health insurance card, or if a patient does not have insurance this information would need to be submitted to the US ********** of ***** and ***** Services HHS program. Also, it helps prevent fraudulent testing. Center for COVID Control manages patient testing for *************************** which is federally licensed for the centers for ******** and ******** services CMS. All of our staff is HIPAA certified.

      All patient information is stored in a safe and secure matter with limited access which is used for future reporting purposes. Center for COVID Control has been in business for over a year and we have not had any type of security issues. Rest assure, if we were to have any such matters we would reach out to all of our patients. Center for COVID Control is a CLIA certified lab. We would not put our patients and lab at risk with any federal rules and requirements.

      Having recently had exposure to someone who had just test positive for Covid, I needed test results before attending a family gathering on Christmas Eve with my elderly parents and other older adults. So, I went to the Center for Covid Center testing site on ******************* in *******. After waiting in line and supplying a test swab and filling out the information card - which I believe required my Social Security Number - I dropped off the plastic bag containing all this to a technician working there requesting an emergency result, one within a half-hour or so. That was about 9:30 AM on December 24. I still have not received the result two weeks later. I did not receive a receipt with my sample and information, which might have been a warning sign. I thought that this might have been due to incompetence on the part of the testing site, but the same thing happened to a friend at the same site.       The website for this company states that they ***** give you results for a Covid test within ***** mins and that is not the case. They have withheld my test results and have no made any attempt to help me locate them. They also claimed I would receive a phone call but have not and Ive tried calling them multiple times. I dont believe this a legitimate Covid testing facility and think they may be collecting customer information for malicious use.

      They scanned my drivers license and never sent me my PCR ********************** test results. They only told me my rapid results verbally. No written confirmation was provided. I went to the location on ********, **.

      As for the requirements of gathering photo identification, it is Doctors Clinical Laboratories policy to verify patients identity that matches the patients health insurance card, or if a patient does not have insurance this information would need to be submitted to the ** ********** of ****** and ***** Services HHS program. Also, this helps prevent fraudulent testing.

      As for your results, I am showing that you took a PCR test on 12/6 and results were sent to you via email and text on 12/11. The email address your results were sent to was *********************************. This is the email address that was provided at the time of testing when the ** code was scanned. 

      I have re-submitted your results to you as well.

      Date: 12/27/2021 | Amount paid: 0 | Business provides: COVID-19 testing | Nature of dispute: potential fake test/results, identity theft and insurance fraud; business will not answer phone Location I went to is working out of an obscure trailer. To register for test you have to scan ** code that they give you when youre in line. They ask for picture of your ID or passport and insurance card. Not providing copy of your ID is not optional. Also, seemed strange that you swab your nose then have to put the swab back in the wrapper.I found a random webpage with de-identified results that look exactly like what I received when I scanned the ** code on my report. Difference is the Date/Time that appear on the report each time you scan. Date of sample collection on my report is incorrect (indicates collected on the next day at 7:39am but they dont open until 9:00am). Attachment 1 & 2 Found Privacy Policy on their website which shouldnt be valid as there is no box to check to agree to those terms. They refer to an app which Im assuming you unknowingly download to your device as you are required to scan the ** code when you register. Attachment 3 I started having computer issues after I randomly found ** code online and received strange email from Amazon when I deleted my account from my iPhone. Attachment 4 I called them twice and pressed #2 for government official and now their website doesnt appear 1st. I drove by site the next day and they are still in the same location but they moved the trailer where its less visible.Their website seems bogus and in searches they appear like they are linked to the government/CDC. I also found a website for ******* Covid Control Center that is very similar to the Center for Covid Control website. Attachment 5 & 6

      This company has set up a pop-up shop for COVID testing (both PCR and rapid) in the next town over from where I live (*******, **). This business is either a scam, or it is simply poorly run and not capable of handling the influx of testing they've gotten. I received both a PCR and rapid test here on 12/17/21 after being exposed to someone with COVID. I never received results for either test. I did receive a confirmation email when I registered, so I know they have my contact information correct. They just never followed through and provided the results for my tests -- either one. When I tried to call the company, I wanted on hold for literally 3 hours, and when I finally spoke to a customer service representative, she actually got frustrated and hung up on me, when I asked to speak to a supervisor because she refused to provide me with the results over the phone. I know that many other customers had similar experiences to me, and never received their results, based on ******** posts on our community ******** group. I was not an isolated incident.We are in the middle of a global pandemic, and not being able to get test results can mean the difference of life and death when it comes to COVID. It's criminal that this company is allowed to operate given their unreliability and lack of customer service. By false advertising and enticing customers to get tested, they are preventing sick or exposed customers from going to a different, legitimate place where they would actually receive their results reliably, and could modify their behavior and quarantine if positive. This place should be investigated by the Government and put out business before they harm more people.
