I visited the Midwest Express Clinic on 02/23/22 for an allergic reaction. I was told that even though my insurance was accepted at the location, the could not process it at the time and Id have to pay out of pocket and submit a reimbursement to my insurance company. I paid $120 using a credit card and was only given a transaction receipt which was not sufficient to submit to the insurance company. I was advised that Id be getting an email and/or physical mail copy with itemized **** in the next few days. I never received it, so I followed up a few days later. On 03/02/22 I was emailed another credit card transaction copy, so I followed up again. On 03/16/22 I was emailed a receipt for an office visit without medical code, which I submitted to insurance and the claim was rejected. I visited the location on 3 different occasions, called 15+ times, but I am still not able to get an itemized receipt with a medical code of services rendered from this place. Multiple associates from the central billing department assured me that Id be getting it in the mail, but Im yet to see it. My account number with them is ******** would like to get a proper receipt, so I could finally get this claim processed by my insurance company.Initial Complaint

I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with Transunion, Equifax, and Experian. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. I hereby exercise my legal rights enacted by Congress and The ************************* which explicitly states when a victim of identity-theft dispute a fraudulent account on his/her credit report it shall be honored by all credit bureaus and all fraudulent information should be blocked within 4 days and proper notification shall be given to all data furnishers pursuant to section 605b of The Federal Fair Credit Reporting Act.1. There has been an ongoing issue with my September 2021 **** due to the business inputting my address in their system incorrectly. They sent my **** to collections and I paid it before realizing what had actually happened. When I called they admitted fault yet told me there is nothing that could be done considering I paid the agency already. A company should not be able to admit fault and then say they can't reimburse the fault. I simply would like to be reimbursed.Initial Complaint

On 19 September 2021, our daughter went to Midwest Express Clinic on *********************************************************. Midwest charged our daughters **** for a covid test on 30 November 2021, instead of our insurance GEHA, despite providing them all her insurance information. This claim should have been sent to our insurance GEHA Elevate Plus for processing before ever billing our credit card, but as of 18 March 2022 they still have not sent GEHA a claim for this covid test. Instead, they charged my credit card $150 without permission on 11/30/2021, and refuse to refund this moneywhile also neglecting to submit the paperwork to GEHA. In the attachment, I have documented my calls with Midwest Express Clinic, asking repeatedly to send this claim to our insurance company. They tell me they will do so, but everytime I check with my insurance company they say they still have never received a **** from Midwest Express Clinic. I believe Midwest Express Clinic has no intention of ever going through my insurance company, since they have made more money by charging me.Business response

Im being charged for medical service which my insurance will pay for and Midwest clinic refuses to provide my insurance company with the correct coding information so the **** can be paid. Midwest has submitted the charge as a debt to my credit report lower my credit score and the **** is not my responsibility its Midwest fault for incorrectly putting in a code . I have been calling since January 2021,2022 5for them to put the correct code and Midwest keep telling me the review will be 7 - 10 business days which have been over the allotted time period.Business response

I never went to this place the date they claim I went there and use my identity as if I was a man when I am a woman to pay off someone else care for that day I got the **** in the mail for ******************************************************************** the insurance didn't take because it states you are a man and for me to call ********** blue shield you have to call them because they said you are a men. I no that this is all a fraudulent thing. I even wrote to them in I timely matter told them this was identity fraud it was not me and they still kept it on file and send this to credit bureaus now it's in collection. They are a very fraudulent company. Needs to be shut down for business.Business response

I visited this clinic for a covid test required for travel. I was quoted $175, I paid, and I was issued a receipt. They proceeded with the test. A week later, I began receiving additional bills for $120 via mail for "Medical Charges." I contacted the clinic, but they said only their billing department could help with that. So I called their billing department, who agreed the charges looked strange, and they agreed to open a dispute. When I called the billing department again over a week later to check the status, they said the charges were still valid because they did not receive any response from the clinic I believe this business is perpetrating a scam which will result in these false bills going to debt collections. They have purposely made their customer service, billing department, and dispute centers difficult and confusing to contact. And I visited with another patient for covid testing who received the EXACT SAME additional bills after completing full payment.Business response

I have contacted both the billing team and the patient.
I explained to the patient via email that I have requested the billing team to remove the charge of $120.00. I have also informed the billing team that the pt has satided the bill on the date of service on 7/18/2021.

I'm requesting that the charge of $120.00 be removed from the patient bill. The pt satisfied the bill on the dos. It looks like this charge was in error. Please make sure this is removed as soons as possible. Make sure this pt is not sent to collections.Customer response
