    I am filing this on behalf of my mother ***********************. She resides at *********************************************** . My mother was sent to symphony after being hospitalized for hip injury. From the minute we walked in the door we were being pushed to send her home from that second. Of which we had no ability to care for her at home. The person who was supposed to be her advocate I am told is now been fired. But that doesn't help anything that happened to us during her stay. At one point during her stay she was supposed to go to therapy and is supposed to have assistance to get the ability to do that that was denied to her and she fell. Took almost 12 hours to get x-rays on her other hip that had not been injured previously. And then those results were denied any information to us being the family or to ***********************. Also the people at symphony did not forward any information to ******** to allow her stay to continue. Upon numerous requests asked for records of that occurring we were also denied. We were forced then to give a cash deposit of $5,000 for her to remain. Once again symphony neglected to send any information to ********. Also within that time frame after we were denied access to her medical records from her fall at symphony we took her to a family doctor. Who then wrote orders for her to stay for at least another 3 to 4 weeks. Symphony once again neglected to send any of that information to ******** forcing us to pay the cash deposit. During that outside visit my brother another family member ********************* was injured in the parking lot because symphony refused to help load and unload my mother out of his vehicle. If symphony would have done what they were supposed to have done forward the information to ******** in the proper manner we would have never had to have paid the access amount. I'm looking for full disclosure from symphony of what they did and didn't do during this visit and the negligent reports on her falling under their care.
