    I am the **************** Proxy for my father, *** C. *******. Through his assisted living facility - ************* in ************, ** - he was set up with counseling through 1st Class Behavioral. *** has ******** and a supplemental, so he should not be receiving bills from 1st Class. In July, In July, I received several invoices and then almost immediately Past Due notices from them. I paid one of them, but then looked into it. It appears they billed both my father and his insurances. I have asked repeatedly for them to send a refund for the payment that was made ($80.89). I've been told that they will take care of it. I have a lengthy email chain that shows this. As of today, we still have not received the refund. While the amount is relatively small, this is for an 83 year old man who has since been moved into skilled nursing and is applying for ********. Every little bit counts and I would like to see him refunded.Invoice #**** for $80.89 Payment (ck#******), dated 7/22/2024, but cashed on 8/28/2024

    On August 12, 2024, we received an inquiry from ********* ***** to review her father's account, who was a patient in our practice,concerning a bill she received and she mailed us for $80.89 on July 29, 2024. The companys ****** reviewed her father's account to confirm that the $80.89 was collected from the patients secondary insurance, which was found to be accurate. The ****** emailed ********* to inform her that we would mail her a check and refunded her the $80.89. After confirming that the credit of $80.89 was due, I emailed ********* on September 10th, 2024, to request the correct address for mailing the check, but I did not receive a response. After reviewing her fathers documents, I was able to locate *********** address, we mailed ********* the check via certified mail on September 20th, 2024. Today, September 23th, 2024, I informed ********* via text that a check for $80.89 had been sent to her through certified mail since Friday 09/20/2024 (see attached text).

    Please attached for the receipt of the certified mail as well as the copy carbon of the check. Thank you.
