On June 29, 2023, SM Mechanical LLC sub-contracted ******** ********** arrived at my address to complete insulation activities including building of a foam box over the elevator motor, cables, top of shaft, as well as blown-in cellulose insulation. SM Mechanical LLC was not on premise during ******** **********’s work activities. ******** ********** sent two (2) workers to complete all insulation activities, one male (worker 1) and one female (worker 2), with associated equipment and materials. Worker 1 and Worker 2 both witnessed the elevator in operation shortly after arrival as it was used to move miscellaneous items from the basement area to facilitate worker access. In the hours between 12 noon and 6pm EST, Worker 1 transitioned to attic spaces to complete insulation activities. Worker 1 did not shut off power to the control box, motor, or cables to the elevator at residence, thus creating a hazardous work area during the construction of the foam insulation box encompassing the elevator motor, cables and top of shaft. Furthermore, Worker 1 did not shut off power to the control box during the activity of installing blown-in cellulose insulation. Before Worker 1 and Worker 2 completed post work pressure test, the residence owner noticed a change in the elevator indication lights (now blinking red and green), indicating error in elevator function. The elevator was unable to respond to call from any floor post Worker 1 foam box and blown-in insulation activities. Worker 1 not only notified his supervisor at ******** ********** but attempted to power cycle elevator associated power supplies in hope of remedying now inoperative elevator at residence. Remedy efforts were unsuccessful in achieving previous operations of the elevator. Since this event, SM Mechanical LLC has indicated they do not feel liable for the damage(s) and has not responded to emails to remedy since last week.
