    I came to this business on 2/28/2024. I needed a TB test performed along with a Statement of Good Health/Free of Communicable Disease form filled out. I showed two employees the form 'before' I filled out any of their paperwork and signed anything and explained to them that I needed the TB test and entire form filled out. They said "yes", this will be done after the test was completed. After I filled out the paperwork, they charged my credit card $40 before the TB test was performed. I found this odd and that usually charges would come after the service. After the TB test was completed, the employee who did the test handed me the form only partially completed. I told her please fill out/have the the rest of the form as was agreed to by two employees when I showed them the form prior to me getting the test. She refused and then told me that I would need to pay an additional $75 for a physical. This was NEVER mentioned to me by either of the two employees when I showed them the form. I deserve a refund on my credit card considering I showed them the form before the service and they both assured me that it would be filled out entirely. They never mentioned to me an additional $75 until after the service was performed. I am seeking a refund of $40.00 on my credit card due to the unprofessionalism and lying.
